摘要
目的:探讨并分析不同微创手术治疗大体积前列腺增生症(BPH)的疗效与预后。方法:收集2012年1月~2016年1月我院诊治的大体积前列腺增生患者140例,分别选择两种不同微创手术方式进行治疗,其中经尿道前列腺电切术(TURP)者76例为TURP组,行腹腔镜下耻骨后保留尿道前列腺切除术者64例为腹腔镜组,对两组患者的术中出血量、手术时间、切除腺体质量、膀胱冲洗时间、带管时间、术后症状评分、生活质量评分等进行比较分析。结果:腹腔镜组患者膀胱冲洗及带管时间比TURP组短,切除腺体重量较TURP组多,差异有统计学意义(P<0.05);腹腔镜组患者术中出血量及手术时间与TURP组比较差异无统计学意义(P>0.05)。两组术后症状评分、生活质量评分较术前显著提高,并发症发病率相差较大,两者差异均有统计学意义(P<0.05);而两组进行组间比较示差异无统计学意义(P>0.05)。结论:两种微创手术方式治疗大体积BPH均能显著缓解症状、提高生活质量,但经腹腔镜治疗者具有无须进行膀胱冲洗、带管时间短及愈合快等特点,值得临床推广应用。
Objective：To compare efficacy and prognosis between two minimally invasive surgical treatments for large volume benign prostatic hyperplasia.Method：We collected 140 cases of large volume benign prostatic hyperplasia patients in our hospital from January 2012 to January 2016.Seventy-six of them underwent transurethral resection of prostate（TURP）,while other 64 patients chose laparoscopic pubic Madigan prostatectomy.Data of blood loss,operative time,the weight of removal of the gland,bladder irrigation time,tube indwelling period,postoperative symptom scores,quality of life scores,etc.were analyzed and compared between two groups.Result：Bladder irrigation time and tube indwelling period of laparoscopic group were shorter than those of TURP group,and removal of the gland was more than the TURP group,which the difference was statistically significant（P0.05）.However,blood loss and operation time between two groups showed no difference（P0.05）.Postoperative symptom score and quality of life score imporved significantly in both two groups compared with the preoperative data.Also,it showed statistically significant difference in complication rate（P0.05）.But no statistically significant difference was found between the two groups（P0.05）.Conclusion：Both two kinds of minimally invasive surgical treatments can significantly relieve symptoms and improve quality of life for patients with large volume benign prostatic hyperplasia.However,laparoscopic treatment is superior and worthy of clinical application for its shorter tube indwelling period,fast recovery and no need of bladder irrigation.
