摘要
目的探讨球囊扩张联合输尿管软镜下钬激光碎石(URHL)治疗肾结石合并输尿管狭窄疗效及对血肌酐(Scr)、尿素氮(BUN)、胱抑素(Cys C)和超敏反应蛋白(hs-CRP)水平的影响。方法我院收治的肾结石合并输尿管狭窄患者86例,根据治疗方法不同,分为A组(球囊扩张+URHL,45例)和B组(筋膜扩张器+URHL,41例),比较两组围术期情况、肾积水情况、结石清除率及术后肾功能、炎症因子水平。结果 A组手术时间和术中出血量均低于B组(P <0. 05),两组一次扩张成功率、住院时间、肾积水有效率和结石清除率比较,差异无统计学意义(P> 0. 05);A组术后Scr、BUN、Cys C、hs-CRP、白介素-1β(IL-1β)、前列腺素E2(PGE2)、过氧化脂(LPO)水平及并发症发生率均低于B组(P <0. 05)。结论球囊扩张联合URHL治疗肾结石合并输尿管狭窄结石清除率较高,并发症少,安全性有保证。
Objective To investigate the efficacy of balloon dilatation combined with ureteroscopic holmium laser lithotripsy(URHL)in the treatment of ureteral stricture with renal calculi and its effects on levels of serum creatinine(Scr),urea nitrogen(BUN),cystatin(CysC)and high-sensitivity C-reactive protein(hs-CRP).Methods Eighty-six patients with renal calculi and ure-teral stricture were enrolled in our hospital were divided into group A(balloon dilatation+URHL,n=45)and group B(fascia dilata-tion+URHL,n=41)according to different treatment methods.The perioperative conditions,hydronephrosis,stone clearance rate and postoperative renal function and inflammatory factors were compared between the two groups.Results The operative time and intraoper-ative blood loss in the group A were lower than those in the group B(P<0.05).There were no significant differences in the success rate of one-time expansion,hospital stay,effective rate of hydronephrosis and stone clearance rate between the two groups(P>0.05).The levels of Scr,BUN,CysC,hs-CRP,interleukin-1β(IL-1β),prostaglandin E2(PGE2)and lipid peroxide(LPO)in the group A after operation were lower than those in the group B(P<0.05).The incidence rate of postoperative complications in the group A was significantly lower than that in the group B(P<0.05).Conclusion Balloon dilatation combined with URHL has high stone clearance rate,few complications and guaranteed safety in the treatment of renal calculi with ureteral stricture.
