摘要
目的通过比较高弓足患者与正常人足底各分区的平均压强和触地面积,为高弓足患者的诊断和治疗提供有价值的生物力学参数。方法本研究共纳入50名成年受试者,其中25人为高弓足,25人为正常足。采用Footscan足底压力测试系统对受试者进行动态足底压力测试,系统自带软件将足底面积划分为拇趾、2~4趾、第1跖骨区、第2跖骨区、第3跖骨区、第4跖骨区、第5跖骨区、足中部、足跟内侧区以及足跟外侧区等10个区域。采用独立样本t检验对两组受试者的数据进行统计学分析。结果高弓足患者总的触地面积较正常组明显减少,同时其第1,第2,第3和第4跖骨区的平均压强明显升高。与正常足相比,高弓足的总接触面积减小,拇趾区的平均压强减少(数值)。载荷主要向内侧跖骨区域转移,但是跖骨区内部的相对载荷分布比例在两组间并无显著差异。结论高弓足人群足底压力参数的特点,增加了足部运动损伤发生概率,在临床工作中,加强高弓足人群足底压力参数的检测、评估、及早期矫正工作,以减轻相应区域疼痛及预防运动损伤的发生。
Objective:The objective of this study is to compare the plantar pressures and weight-bearing surface in a population with pes cavus to a population with neutral feet,so as to provide valuable references for clinical diagnosis and therapy of these patients.Methods The sample comprised 50 adults,25 with pes cavus and 25 with neutral feet.The Footscan pressure plate was used to measure the total contact area and plantar pressure under the lateral and medial heels,mid-foot,first,second,third,fourth and fifth metatarsal heads,hallux,second to fifth toes.A statistical analysis of the data was performed using Student’s t-test for independent samples.Results The pes cavus subjects showed a significant reduction in their weight-bearing area and significantly increased mean pressures under the first to the fourth metatarsals.Compared to neutral feet,pes cavus feet show a reduction in total contact surface and the mean pressure under the hallux.A significant increase is present in the load under the medial metatarsal areas,but the relative distribution of the load under the metatarsal heads is similar in both groups.Conclusions The characteristics of plantar pressure in pes cavus population increase the probability of foot sports injures,In the future clinical practice,we should focus on monitoring,assessing the plantar pressure in pes cavus population and treating the areas bearing excessive load,to reduce the pain of corresponding areas and prevent the occurrence of sports injuries.
