摘要
目的探讨脊柱关节炎(SpA)患者人类白细胞抗原-B27(HLA-B27)及炎症指标的变化及其临床意义。方法选取2014年1月~2018年12月我院收治的符合研究条件的196例SpA患者作为SpA组,另选取同期在我院进行检查的200例健康体检者作为健康对照组。检测血液HLA-B27、肿瘤坏死因子-α(TNF-α)、C-反应蛋白(CRP)及泌尿系彩超、腹部立位X线片。观察比较两组研究对象的尿石症发生情况,比较SpA组尿石症(+)患者和SpA组尿石症(-)患者的HLA-B27阳性率及TNF-α、CRP指标水平。结果SpA组的尿石症发生率为19.89%(35/176),高于健康对照组的3.00%(6/200),差异有统计学意义(P<0.01)。SpA组尿石症(+)患者的HLB-B27阳性率(82.86%)高于尿石症(-)患者(60.99%),差异有统计学意义(P<0.05)。SpA组尿石症(+)患者的TNF-α、CRP水平均高于尿石症(-)患者,差异有统计学意义(P<0.05)。结论SpA患者尿石症发生率高,HLA-B27、TNF-α、CRP对尿石症发生风险有一定的预测作用。
Objective To investigate the changes of human leukocyte antigen-B27 and inflammatory indexs in patients with spondyloarthritis(SpA)and its clinical significance.Methods A total of 196 cases of SpA patients admitted to our hospital from January 2014 to December 2018,which met the research conditions,were selected as the SpA group,and 200 cases of healthy physical examination patients who were examined in our hospital during the same period were selected as the healthy control group.Blood HLA-B27,tumor necrosis factor-α(TNF-α),C-reactive protein(CRP),urinary color doppler ultrasound and abdominal orthotopic X-ray were detected.The occurrence of urolithiasis in the two groups was observed and compared.HLA-B27 positive rate,TNF-αand CRP levels of patients with urolithiasis(+)SpA group and patients with urolithiasis(-)SpA group were compared.Results The incidence of urolithiasis in SpA group was 19.89%(35/176),which was higher than 3.00%(6/200)in healthy control group,the difference was statistically significant(P<0.01).The HLA-B27 positive rate in urolithiasis(+)SpA group(82.86%)was higher than that in urolithiasis(-)SpA group(60.99%),the difference was statistically significant(P<0.05).The levels of TNF-αand CRP in urolithiasis(+)SpA group were higher than those in urolithiasis(-)SpA group,the differences were statistically significant(P<0.05).Conclusion The incidence of urolithiasis in patients with SpA is high,the HLA-B27,TNF-α,and CRP have a certain predictive effect on the risk of urolithiasis.
