摘要
目的探讨手术治疗急性闭角型青光眼合并白内障的疗效与安全性。方法择取2016年10月—2019年4月该院眼科78例单眼急性闭角型青光眼合并白内障患者为研究对象,随机分为实验组(Phaco+iol植入联合小梁切除术,39例)和对照组(Phaco+iol植入术联合房角分离术/周边虹膜激光术,39例),术后随访,比较观察两组疗效,统计安全性。结果实验组术后矫正视力(0.45±0.06),眼压(16.7±4.2)mmHg,并发症率5.13%,中央前房深度(3.19±0.22)mm,房角(3.55±0.43)°,与对照组比较差异无统计学意义(t=0.182、0.643,χ^2=2.052,t=2.281、2.974,P>0.05)。结论Phaco+iol植入联合小梁切除术或房角分离术/周边虹膜激光术均能有效降低眼压,改善视力,并发症少且轻,应用安全有效,临床可根据患者实际酌情选择手术方式。
Objective To evaluate the efficacy and safety of surgical treatment of acute angle-closure glaucoma with cataract.Methods From October 2016 to April 2019,78 patients with monocular acute angle-closure glaucoma with cataract in the hospital were randomly divided into experimental group(Phaco+iol implantation combined with trabeculectomy,39 cases).The control group(Phaco+iol implantation combined with angle separation/peripheral iridolithography,39 cases),follow-up,compare the efficacy of the two groups,statistical safety.Results The corrected visual acuity(0.45±0.06),intraocular pressure(16.7±4.2)mmHg,complication rate 5.13%,central anterior chamber depth(3.19±0.22)mm,angle angle(3.55±0.43)°in the experimental group compared with that in the control group,and the difference was statistically significant(t=0.182,0.643,χ^2=2.052,t=2.281,2.974,P>0.05).Conclusion Phaco+iol implantation combined with trabeculectomy or angle separation/peripheral irradiance can effectively reduce intraocular pressure,improve visual acuity,less complications and lighter,safe and effective application,and clinically select surgical methods according to the actual situation of patients.
