摘要
目的分析呼吸机辅助通气全身麻醉下纤维支气管镜取儿童支气管异物的临床效果。方法选取在本院治疗的支气管异物患儿20例为观察对象,所有患儿均在全身麻醉呼吸机辅助通气下,经纤维支气管镜取异物,对患者治疗效果做回顾性分析。结果本组20例患儿中,16例患儿一次成功取出异物,2例患儿经2次操作后成功取出异物,另外2例患儿经钳钳碎异物后灌洗完成治疗,异物取出成功率为100.00%。耐受性达到优标准15例,所占比重75.00%,耐受性达到良标准5例,所占比重25.00%,异物取出耐受优良率为100.00%。异物取出过程清晰15例,所占比重75.00%,取出过程比较清晰4例,所占比重20.00%,取出过程不清晰1例,所占比重5.00%。结论儿童支气管异物与成年人不同,在行纤维支气管镜取出术时无法获取到患儿的良好配合,为此,需要有效手段促使异物取出术能够顺利进行。呼吸机辅助通气全身麻醉的应用,可促使患儿在进入到睡眠状态后实施异物取出操作,取出成功率较高,且清晰度与耐受性良好。
Objective To analyze the clinical effect of fiberoptic bronchoscopy on children’s bronchial foreign body under ventilator assisted ventilation and general anesthesia.Methods Twenty cases of children with bronchial foreign body treated in our hospital were selected as the observation objects.All the children were treated with general anesthesia ventilator assisted ventilation,and the foreign body was taken through fiberoptic bronchoscopy,and the treatment effect of the patients was retrospectively analyzed.Results Of the 20 cases in this group,16 cases were successfully removed the foreign body at one time,2 cases were successfully removed the foreign body after 2 operations,and the other 2 cases were successfully treated by lavage after crushing the foreign body with forceps.The success rate of foreign body removal was 100.00%.Tolerance reached the excellent standard in 15 cases,accounting for 75.00%;tolerance reached the good standard in 5 cases,accounting for 25.00%;and tolerance of foreign body removal was 100.00%.The removal process was clear in 15 cases(75.00%),clear in 4 cases(20.00%)and unclear in 1(5.00%).Conclusion Children with bronchial foreign bodies are different from adults.Good cooperation cannot be obtained during fiberoptic bronchoscopy removal.Therefore,effective means are needed to facilitate the smooth removal of foreign bodies.The application of ventilator-assisted ventilation and general anesthesia can promote the removal of foreign bodies in children after they enter the sleep state,with a high success rate,clarity and good tolerance.
