摘要
目的探讨有创呼吸机辅助通气下纤维支气管镜灌洗治疗小儿重症肺炎的效果。方法选择2016年5月-2017年4月在四川省妇幼保健院住院治疗的52例小儿重症肺炎患者为研究对象,随机分为观察组和对照组,每组各26例。观察组实施有创呼吸机辅助通气下纤维支气管镜灌洗治疗,对照组单纯采用有创呼吸机辅助通气治疗。观察两组治疗效果,并对比观察组治疗前和治疗后48 h患儿的血气变化情况。结果观察组治疗总有效率为92.86%,对照组为91.67%,两组比较,差异有统计学意义(χ~2=6.702,P=0.035)。观察组治疗48 h后患儿的动脉血氧分压(PaO_2)、动脉血二氧化碳分压(PaCO_2)和氧合指数(PaO_2/FiO_2)均优于治疗前(P<0.05)。观察组治疗后的总氧疗时间、呼吸机使用时间均对照组(P<0.05)。结论有创呼吸机辅助通气下纤维支气管镜灌洗治疗小儿重症肺炎效果显著。
Objective To explore the curative effect of bronchofibroscopic lavage under invasive mechanical ventilation in treatment of severe pneumonia in children. Methods Fifty-two children with severe pneumonia treated in Sichuan Provincial Maternal and Child Health Care Hospital from May 2016 to April 2017 were selected and randomly divided into observation group and control group,52 children in each group. The children in observation group were treated by bronchofibroscopic lavage under invasive mechanical ventilation,and the children in control group were treated by invasive mechanical ventilation. The curative effects in the two groups were observed. The changes of blood gas indexes before treatment and at 48 hours after treatment in observation group were compared. Results The total effective rates in observation group and control group were 92. 86% and 91. 67%,respectively,there was statistically significant difference between the two groups（ χ~2=6. 702,P = 0. 035）. In observation group,PaO_2,PaCO_2,and PaO_2/FiO_2 at 48 hours after treatment were statistically significantly better than those before treatment（ P0. 05）. The total time of oxygen therapy and the time of ventilator after treatment in observation group was statistically significantly shorter than that in control group（ P0. 05）. Conclusion The curative effect of bronchofibroscopic lavage under invasive mechanical ventilation in treatment of children with severe pneumonia is significant.
