摘要
目的:探究茵栀黄注射液治疗肝移植术后高胆红素血症对患者同型半胱氨酸(Hcy)和γ-谷氨酸转肽酶(γ-GT)的影响。方法:选取2015年1月至2018年1月上海长征医院收治的肝移植术后高胆红素血症患者80例作为研究对象,按随机数字表法随机分为对照组和观察组,每组40例。对照组在抗排斥、抗病毒的基础上给予优思弗药物治疗,观察组在此基础上给予茵栀黄注射液治疗,2组均连续治疗4周。统计2组治疗后临床疗效;比较2组患者治疗前后肝功能指标如直接胆红素(DBiL)、丙氨酸氨基转移酶(ALT)、天冬氨酸转氨酶(AST)、血清总胆红素以及Hcy、γ-GT的差异。结果:治疗后观察组与对照组的治疗有效率分别为95.00%、75.00%,观察组显著高于对照组,差异有统计学意义(P<0.05);与治疗前比较,治疗后4周血清总胆红素值显著降低,且观察组显著低于对照组,差异有统计学意义P<0.05);治疗后4周肝功能指标DBiL、ALT、AST显著降低,且观察组显著低于对照组,差异有统计学意义(P<0.05);治疗后4周Hcy、γ-GT指标显著降低,且观察组显著低于对照组,差异有统计学意义(P<0.05)。结论:茵栀黄注射液可有效缓解肝移植术后高胆红素血症临床症状,对肝功能指标(DBiL、ALT、AST)、血清总胆红素以及Hcy、γ-GT等方面有显著改善,且疗效优于优思弗治疗。
Objective:To explore the effects of Yinzhihuang Injection on Hcy andγ-GT in patients with hyperbilirubinemia after liver transplantation.Methods:A total of 80 patients with hyperbilirubinemia after liver transplantation admitted to Shanghai Changzheng Hospital from January 2015 to January 2018 were divided into a control group(n=40)and an observation group(n=40)according to random number table method.The control group was treated with Yousifu on the basis of anti-rejection and anti-virus.The observation group was treated with Yinzhihuang Injection on the basis of the control group.Both groups were treated for 4 weeks.The clinical efficacy of the 2 groups was counted,and the differences of liver function indexes(DBIL,ALT,AST),serum total bilirubin,Hcy andγ-GT between the 2 groups before and after treatment were compared.Results:After treatment,the clinical total effective rates of the observation group and the control group were 95.00%and 75.00%,and the observation group was significantly higher than the control group(P<0.05).Compared with before treatment,the serum total bilirubin values of the observation group were significantly lower than those of the control group(P<0.05),and the changes of DBIL,ALT and AST in the liver function indexes of the observation group were significantly lower than those of the control group(P<0.05).The indexes of Hcy andγ-GT in the observation group were significantly lower than those in the control group 4 weeks after treatment(P<0.05).Conclusion:Yinzhihuang Injection can effectively alleviate the clinical symptoms of hyperbilirubinemia after liver transplantation.It can significantly improve the indexes of liver function(DBIL,ALT,AST),serum total bilirubin,Hcy andγ-GT,and has better curative effects than Yousifu therapy.It is worthy of clinical application.
