摘要
目的总结生长发育迟缓合并糖尿病患者的临床诊治经验。方法选择2013年9月17日就诊于山东大学齐鲁医院的1例生长发育迟缓合并糖尿病患者,对其临床特点、诊治经过、诊治思路进行回顾分析。结果患者女性,15岁。入院后结合体格检查,完善胰岛功能、生长激素激发试验、垂体MRI等检查,最终通过基因检测诊断为Prader-Willi综合征(PWS)。结论对于生长发育迟缓合并糖尿病的患者应警惕特殊类型糖尿病可能,详细的专科查体和有针对性的检查十分必要,动态观察患者的临床表现、功能试验,必要时结合基因检测,对于提高诊断准确性非常重要。
Objective To summarize the diagnosis and treatment of growth retardation and diabetes mellitus(DM).Methods The clinical features, laboratory examination, gene analysis of one patient with growth retardation and DM were retrospectively analyzed. Results The female patient was 15 years old, complained of growth retardation for eight years and high blood gulcose for two years. The physical examination showed special clinical features including almond shaped eyes and small hands and feet, labortary examination showed high blood gulcose with insulin resistence, subclinical hypothyroidism, uterine dysplasia with normal chromosome, and moderate fatty liver. Finally Prader-Willi syndrome was confirmed by gene analysis. Conclusions Growth retardation combined with DM is a difficult clinical problem. It highly indicates a special type of DM with syndrome disease. Carefully physical examination and labortory examination are important to successful diagnosis.
