摘要
目的探讨强直性脊柱炎(AS)患者外周血血浆中miR-223和基质金属蛋白酶(MMP)-3、MMP-9的表达水平及其临床意义。方法随机选取48例AS患者作为观察组,选取同期健康体检者48例作为对照组,采用实时逆转录聚合酶链式反应及酶联免疫吸附试验检测miR-223、MMP-3及MMP-9的表达。应用SPSS17.0软件绘制检测指标的受试者工作特征曲线(ROC曲线),对其诊断价值进行分析评价。结果观察组患者外周血中miR-223、MMP-3及MMP-9的表达均高于对照组,在疾病活动期表达水平均有不同程度的升高。血浆中miR-223、MMP-3及MMP-9之间的表达存在一定相关性(P<0.01),也与患者疾病活动指数、人体白细胞抗原-B27、C反应蛋白及红细胞沉降率等指标均存在一定相关性(P<0.01)。ROC曲线分析表明,miR-223、MMP-3及MMP-9对AS具有较高的诊断敏感度和特异度,联合检测能提高临床对AS的诊断能力。结论外周血miR-223、MMP-3及MMP-9在AS患者中存在差异表达,并具有一定的临床诊断能力,有望成为新的生物学诊断指标。
Objective To investigate the expression levels of miR-223,matrix metalloproteinase(MMP)-3 and MMP-9 in peripheral blood plasma of patients with ankylosing spondylitis(AS)and its clinical significance.Methods Forty-eight AS patients were randomly selected as a case study group,and 48 healthy subjects for the same period were selected as a healthy control group.Real-time fluorescent quantitative polymerase chain reaction and enzyme-linked immunosorbent assay were used to detect the expression of miR-223,MMP-3 and MMP-9.SPSS17.0 software was used to draw the receiver's working characteristic curve(ROC curve)of the detection indicators,and its diagnostic value was analyzed and evaluated.Results The expressions of miR-223,MMP-3 and MMP-9 in the peripheral blood of patients in the case study group were higher than those in the healthy control group,and the expression levels increased in varying degrees during the disease active phase.There was some correlation between the expression of miR-223,MMP-3 and MMP-9 in plasma(P<0.01).The three were also related to indicators such as patient disease activity index,human leukocyte antigen-B27,C-reactive protein,and erthrocyte sedimentation rate(P<0.01).ROC curve analysis showed that miR-223,MMP-3 and MMP-9 have higher diagnostic sensitivity and specificity for AS,and combined detection can improve the clinical diagnosis of AS.Conclusion Peripheral blood miR-223,MMP-3 and MMP-9 are differentially expressed in patients with AS and have certain clinical diagnostic capabilities,which are expected to become new biological diagnostic indicators.
