灵活就业人员医保参保之“困” 被引量:17

摘要 应保尽保作为基本医疗保险制度发展的基本要求,不仅关系到制度的可持续,也是实现制度高质量发展的基础条件。然而,目前灵活就业者医保参保率偏低,部分灵活就业人员由于参保面临多重困境而游离于制度之外。参保条件受限、缴费负担过重、转移接续困难是阻碍灵活就业者参加职工医保的主要原因。虽然异地灵活就业者可以选择参加户籍地的居民医保,但是在参保及看病就医过程中存在较多不便。两项制度并行,却难以选择合适的制度参保,由此产生了灵活就业者的漏保现象。制度的优化是破解灵活就业者参保困境的重要途径,可从打破户籍条件的限制、制定适宜的缴费政策、提升医保制度的便携性三方面着手,推动医保制度应保尽保的实现。 The universal coverage of basic medical insurance is an important part in the process of system development,it is not only related to the sustainability of the system,but also the basic condition for realizing the high-quality development of the system.However,at present,the participation rate of fl exible employees in worker’medical insurance is low,and some fl exible employees are out of the system due to multiple diffi culties.The main reasons that prevent fl exible employees from participating in medical insurance are limited conditions,heavy burden of payment and diffi culty in transferring and succeeding.Although fl exible employees in diff erent places can choose to participate in the urban and rural medical insurance,there are many inconveniences of participating medical insurance and seeing a doctor.The two systems are in parallel,but it is diffi cult to choose the appropriate system to participate in,it leads to some fl exible employees miss the medical insurance.Optimizing the system is an important way to break the predicament of the fl exible employees to participate in the medical insurance.We can solve the problem from three aspects:breaking the restrictions of household registration conditions,making the appropriate payment policy and improving the portability of the medical insurance system.These measures will promote the realization of the full coverage of the medical insurance system.
