摘要
目的:探讨25羟维生素D[25(OH)D]水平与慢性肾脏病(CKD)患者肾性贫血及血管钙化的相关性。方法:选取2016年3月至2018年3月安徽医科大学附属阜阳医院收治的126例CKD患者,肾性贫血及血管钙化患者68例为研究组,其余58例为对照组。同时选取同期健康者40例作为健康组。检测各组血清25(OH)D、血红蛋白、促红细胞生成素(EPO)、血清铁蛋白、血钙、血磷、低密度脂蛋白(LDL-C)和高密度脂蛋白(HDL-C)水平。分析25(OH)D与各项生化指标间的关系,并采用受试者工作特征(ROC)曲线分析25(OH)D对血管钙化和肾性贫血的诊断价值。结果:健康组与对照组血清25(OH)D水平比较无明显差异(P>0.05),研究组血清25(OH)D水平显著低于对照组(P<0.05)。3组血钙、血磷、LDL-C和HDL-C比较均无明显差异(均P>0.05)。研究组25(OH)D和血红蛋白水平均显著低于对照组和健康组,而EPO和血清铁蛋白显著高于对照组和健康组(均P<0.05)。研究组血清25(OH)D与血红蛋白呈正相关关系,与EPO和铁蛋白呈负相关关系(均P<0.05),而与血钙、血磷、LDL-C和HDL-C水平无明显相关关系(均P>0.05)。血清25(OH)D诊断CKD患者肾性贫血及血管钙化的ROC曲线下面积为0.911,临界值为14 ng/mL。结论:血清25(OH)D水平在CKD合并肾性贫血和血管钙化患者中显著降低,对于肾性贫血及血管钙化有一定的诊断价值。
Objective:To investigate the relationship between 25-hydroxy-vitamin D[25(OH)D]levels and renal anemia and angiosteosis in patients with chronic kidney disease(CKD).Methods:A total of 126 patients with CKD in our hospital from March 2016 to March 2018 were enrolled.Among them,68 patients with renal anemia and vascular calcification were selected as a study group,and other 58 patients were defined as a control group.40 healthy people were selected as a healthy group.The 25(OH)D,hemoglobin,erythropoietin(EPO),ferritin,calcium,phosphorus,low-density lipoprotein(LDL-C)and high-density lipoprotein(HDL-L)levels in serum were detected.The relationship between 25(OH)D and various biochemical indicators as well as the diagnosis value of 25(OH)D for vascular calcification and renal anemia were analyzed.Results:There was no significant difference in serum 25(OH)D level between the healthy group and the control group(P>0.05).The serum 25(OH)D level in the study group was significantly lower than that in the control group(P<0.05).There was no significant difference in serum calcium,phosphorus,LDL-C and HDL-C levels among the three groups(P>0.05).The 25(OH)D and hemoglobin levels in the study group were significantly lower than those in the control group and healthy group,while EPO and ferritin levels were significantly higher(P<0.05).Serum 25(OH)D was positively correlated with hemoglobin and negatively correlated with EPO and ferritin in the study group(P<0.05),but not correlated with serum calcium,phosphorus,LDL-C and HDL-C(P<0.05).The area under the ROC curve of serum 25(OH)D was 0.911 with a cutoff value of 14 ng/mL.Conclusion:The serum 25(OH)D in CKD patients with renal anemia and vascular calcification is significantly reduced,and it has certain diagnostic value for renal anemia and vascular calcification.
