摘要
目的:通过对寒凝血瘀证和气滞血瘀证这两种临床常见的原发性痛经证型的研究,找出这两种不同证型的临床特征,为临床明确其诊断提供相关临床依据。方法:选择符合标准的寒凝血瘀证和气滞血瘀证原发性痛经患者各48例,分别观察两种证型的VAS评分、CMSS评分、痛经症状评分、疼痛持续时间和生存质量评分。分析比较两种不同证型原发性痛经的临床特征及生存质量的差异。另选择48例健康人(无痛经病史)进行生存质量问卷调查。结果:①两种证型在疼痛持续时间方面无明显差异(均P﹥0.05):②寒凝血瘀证VAS评分、CMSS评分、痛经症状评分均高于气滞血瘀证,且差异具有统计学意义(均P﹤0.05):③两种证型在各领域的生存质量积分方面,差异无统计学意义(P﹥0.05),而与正常健康人群相比较,存在显著性差异(均P﹤0.05),且正常健康人群各领域积分明显高于寒凝血瘀证和气滞血瘀证。结论:寒凝血瘀证与气滞血瘀证相比较,疼痛感更强。疼痛会影响患者的生活质量,两种证型的生存质量较健康人群要差,但两证型之间并无明显差异。
Objective:To approach the clinical characteristics of two different syndrome types of primary dysmenorrhea by studying two clinical common syndrome types of primary dysmenorrhea i.e.cold-coagulation bloodstasis syndrome and qi-stagnation blood-stasis syndrome,in order to provide relevant clinical basis for clinical clear diagnosis.Methods:From patients with primary dysmenorrhea,according to standards of cold-coagulation bloodstasis syndrome and qi-stagnation blood-stasis syndrome,48 cases of patients were selected respectively.The scores of VAS,CMSS,dysmenorrhea symptom,pain duration and life-quality were observed respectively in two syndrome types.The differences of clinical characteristics and life-quality were analyzed and compared between two different syndrome types of primary dysmenorrhea.Other 48 cases of healthy people(without history of dysmenorrhea)were selected for questionnaire of life-quality.Results:1.There was no significant difference in pain duration between two syndrome types(P﹥0.05).2.The scores of VAS,CMSS and dysmenorrhea symptom of cold-coagulation bloodstasis syndrome were all higher than those of qi-stagnation blood-stasis syndrome,and the difference was statistically significant(P﹤0.05).3.In the score of life-quality in each field,the difference between two syndrome types was without statistical significance(P﹥0.05);but compared with healthy people,there was significant difference(P﹤0.05),and the score in each field of normal healthy people was significantly higher than that of cold-coagulation blood-stasis syndrome and qi-stagnation blood-stasis syndrome.Conclusion:Compared cold-coagulation bloodstasis syndrome with qi-stagnation blood-stasis syndrome,the pain perception of cold-coagulation blood-stasis syndrome is stronger.The pain can affect people's life-quality.The life-quality of two syndrome types is worse than that of healthy people,but there is no significant difference between two syndrome types.
