实时三维超声心动图联合二维斑点追踪技术对青年男性力量型运动员右室收缩功能的研究 被引量:10

摘要 目的应用实时三维超声心动图(RT-3DE)联合二维斑点追踪技术(2D-STE)探讨青年男性力量型运动员右室功能变化。方法选取青年男性力量型运动员(运动员组)和健康男性青年(对照组)各30例,应用常规超声心动图测量心率、右房左右径(RA-D1)、右房上下径(RA-D2)、右室基底段舒张末期左右径(RV-D1)、右室中间段舒张末期左右径(RV-D2)、右室舒张末期上下径(RV-D3)、肺动脉收缩压(PASP);应用2D-STE测量右室整体纵向应变(RV-GLS);应用RT-3DE测量右室舒张末期容积(RV-EDV)、右室收缩末期容积(RV-ESV)、每搏输出量(RV-SV)及右室射血分数(RVEF)。分析以上两组数据之间的差异。结果运动员组较对照组RA-D1、RA-D2、RV-D2、PASP、RV-EDV、RV-ESV、RV-SV增高,心率、RVGLS减低,差异均有统计学意义(P <0.05)。两组RV-D1、RV-D3、RVEF差异无统计学意义(P> 0.05)。结论长期参加高强度训练的青年男性力量型运动员右室收缩功能出现亚临床改变;RT-3DE及2D-STE可敏感检测这一变化,为临床评估运动员右心室功能提供可靠依据。 Objective To assess ventricular systolic function in young male strength athletes by real-time three-dimensional echocardiography(RT-3 DE)and two-dimensional speckle tracking echocardiography(2D-STE).Methods A total of 30 young male strength athletes(athlete group)and 30 healthy men(controls group)were selected. Routine echocardiography was applied to obtain heart rate,right atrial left and right diameter(RA-D1),right atrial upper and lower diameter(RA-D2),right ventricular basal segment end diastolic right and left diameter(RV-D1),right ventricular end diastolic middle segment right and left diameter(RV-D2),right ventricular end diastolic upper and lower diameter(RV-D3),pulmonary artery systolic blood pressure(PASP). 2D-STE was applied to obtain right ventricular global longitudinal strain(RV-GLS). RT-3 DE was applied to obtain right ventricular end diastolic volume(RV-EDV),right ventricular end systolic volume(RV-ESV),right ventricular stroke volume(RV-SV),right ventricular ejection fraction(RV-EF).To analyze the differences between the above two sets of data. Results There were a significant increase in RA-D1,RA-D2,RV-D2,PASP,RV-EDV,RVESV,RV-SV and significant decrease in heart rate,RV-GLS in athlete group compared with the controls group(P <0.05). However,there was no significant difference in RV-D1,RV-D3,RVEF between two groups(P > 0.05).Conclusions Subclinical changes of right ventricular systolic function were found in young male strength athletes who participated in high-intensity training for a long time. RT-3DE and 2D-STE can assess this change sensitively and provides reliable information for clinical assessment of right ventricular function in athletes.
