摘要
目的探讨影响高强度聚焦超声治疗外阴上皮内非瘤样病变临床疗效的相关因素。方法采用高强度聚集超声治疗186例外阴上皮内非瘤样变患者并评价治疗效果,统计分析瘙痒部位、瘙痒程度、年龄、病程、病理类型、焦虑和(或)抑郁情况与疗效的关系。结果经高强度聚焦超声治疗,患者的外阴形态基本恢复,瘙痒情况显著改善,总有效率达95.16%。不同的瘙痒部位和程度、不同年龄、不同病程、不同病理类型、不同焦虑和(或)抑郁情况之间治愈率差异有统计学意义(χ^2=35.71,P <0.01;χ^2=41.97,P <0.01;χ^2=10.91,P <0.05;χ^2=21.04,P <0.01;χ^2=10.44,P <0.05;χ^2=16.12,P <0.01)。结论高强度聚焦超声治疗外阴上皮内非瘤样病变的治疗效果不仅与患者的瘙痒程度、年龄、病程及病理类型有关,还与其病灶分布、焦虑和(或)抑郁情况关系密切。
Objective To investigate the factors influencing the clinical efficacy of high-intensity focused ultrasound in the treatment of non-neoplastic lesions of the vulva. Methods High-intensity focused ultrasound was used to treat 186 patients with abnormal intraepithelial neoplasia,and the therapeutic effect was evaluated. And the relationship between itching site,itching degree,age,course of the disease,pathological type and anxiety and/or depression level and curative efficacy were statistically analyzed. Results After high-intensity focused ultrasound treatment,the vulva morphology of the patient returned to normal,and the itching symptoms were relieved.The total effective rate was 95.16%. There were significant differences in cure rates between different itch sites,degrees of itch,ages,courses of disease,pathological types,anxiety and/or depression levels(χ^2=35.71,P <0.01;χ^2=41.97,P < 0.01;χ^2=10.91,P < 0.05;χ^2=21.04,P < 0.01;χ^2=10.44,P < 0.05;χ^2=16.12,P <0.01). Conclusion The therapeutic effect of high-intensity focused ultrasound on non-neoplastic lesions in vulvar epithelium is not only related to the degree of itching,age,course of the disease and pathological type,but also closely related to the distribution of lesions,anxiety and/or depression.
