摘要
目的探讨助产士门诊产前教育对提高围生期孕妇健康素养是否优于仅接受产科医生门诊管理及教育。方法采用队列研究的方式,以定期参加助产士门诊教育为暴露因素,选择2018年1~12月在西南医科大学附属医院产科门诊自愿定期接受助产门诊教育的102例孕妇为观察组,同期仅接受产科医生门诊管理及教育的206例孕妇为对照组。观察两组孕妇围生期健康素养得分和具备健康素养人数比的差异,并分析影响健康素养提高程度的因素。结果入院待产时观察组孕妇的围生期健康素养总分及各领域得分均高于对照组(P<0.05),观察组具备健康素养的人数多于对照组(RR=5.44,P<0.05),观察组孕妇两次围生期健康素养总得分的差值及各领域得分的差值均高于对照组(P<0.05);二元Logistic回归检验发现,定期参加助产士门诊是影响围生期孕产妇健康素养水平提高程度的独立因素。结论助产士门诊产前教育可以增进孕妇对母婴健康知识的了解,提高孕妇围生期健康素养水平,促使其积极参与和配合各种医疗保健活动,值得临床推广应用。
Objective To exploring whether the prenatal education of midwife clinics improving the health literacy of perinatal women better than only receiving the obstetrics outpatient management and education. Methods A cohort study was conducted to regularly participate in midwifery outpatient education as an exposure factor. 102 pregnant women who received voluntary regular outpatient clinic education at the obstetrics clinic of Affiliated Hospital of Southwest Medical University from January to December 2018 were included in the observation group. 206 pregnant women who received obstetrician outpatient management and education were included in the control group. The differences in perinatal health literacy scores and health literacy ratios between the two groups were observed, and factors affecting the improvement of health literacy were analyzed. Results The perinatal health literacy scores and the scores of the fields in the observation group were higher than those in the control group(P<0.05). The number of healthy literacy in the observation group was higher than that in the control group(RR=5.44,P<0.05), the differences between the two perinatal health literacy scores and the scores of the fields in the observation group were higher than those in the control group(P<0.05);the binary logistic regression test found that regular attendance at the midwife clinic was an independent factor in the improvement of maternal health literacy levels. Conclusion Prenatal education in midwifery clinics can improve the understanding of maternal and child health knowledge, improve the health literacy level of pregnant women during perinatal period, and encourage them to actively participate in and cooperate with various health care activities, which is worthy of clinical application.
