摘要
目的:观察针刺联合本体感觉训练治疗慢性踝关节不稳(CAI)的临床疗效。方法:将92例CAI患者按随机数字表法分为对照组和观察组各46例。对照组采用超短波加本体感觉训练治疗,观察组在对照组治疗基础上给予针刺治疗,2组均连续治疗8周。采用踝-后足评分系统(AOFAS)评价患者的踝-后足功能,进行踝关节主动关节位置觉(JPSA)、被动关节位置觉(JPSP)和运动觉(KT)评价,记录疼痛视觉模拟评分法(VAS)评分。以上指标治疗前后各评价1次。结果:治疗后,2组AOFAS评分、关节活动度评分均较治疗前升高(P<0.01),VAS评分均较治疗前降低(P<0.01);观察组AOFAS评分、关节活动度评分均高于对照组(P<0.01),VAS评分低于对照组(P<0.01)。治疗后,2组JPSA、JPSP和KT测量值均较治疗前下降(P<0.01),观察组JPSA、JPSP和KT测量值均小于对照组(P<0.01)。经秩和检验,观察组踝-后足功能优于对照组,差异有统计学意义(P<0.01)。结论:针刺联合本体感觉训练治疗CAI,可以改善患者的本体感觉,减轻疼痛,增加关节活动度,起到改善踝关节功能的作用。
Objective:To observe the clinical effect of acupuncture combined with proprioception enhancement for chronic ankle instability(CAI).Methods:A total of 92 cases of CAI patients were divided into the control group and the observation group according to the random number table method,46 cases in each group.The control group was treated with ultrashort wave combined with proprioception enhancement,and the observation group was additionally treated with acupuncture based on the treatment of the control group.Both groups were treated for eight weeks.The American Orthopaedic Foot and Ankle Society(AOFAS)Ankle-Hindfoot Scale was adopted to evaluate the ankle-hindfoot function;the joint position sense:active(JPSA),joint position sense:passive(JPSP)and Kinaesthesia(KT)in the ankle were evaluated;the scores of Visual Analogue Scale(VAS)were recorded;all the above indexes were evaluated once before and after treatment respectively.Results:After treatment,the scores of AOFAS and range of motion in the two groups were increased when compared with those before treatment(P<0.01),and the scores of VAS were decreased when compared with those before treatment(P<0.01);the scores of AOFAS and range of motion in the observation group were higher than those in the control group(P<0.01),and the score of VAS was lower than that in the control group(P<0.01).After treatment,the measured values of JPSA,JPSP and KT in the two groups were decreased when compared with those before treatment(P<0.01),and the above values in the observation group were lower than those in the control group(P<0.01).The ankle-hindfoot function in the observation group was superior to that in the control group,the difference being significant(P<0.01).Conclusion:The therapy of acupuncture combined with proprioception enhancement for CAI can improve proprioception of patients,relieve pain and increase range of motion,thus playing a role in improving the ankle function.
