谷氨酰胺治疗重症手足口病对患儿体液及细胞免疫功能的影响 被引量:3

摘要 目的探讨谷氨酰胺治疗重症手足口病对患儿体液及细胞免疫功能的影响.方法选取96例重症手足口病患儿,随机分为3组:C组(不含谷氨酰胺肠内营养组,32例),L组(低剂量0.2 g/(kg?d)谷氨酰胺肠内营养组,32例),H组(高剂量0.4 g/(kg?d)谷氨酰胺肠内营养组,32例),于肠内营养支持的第1天、第8天、第15天采集空腹晨血,记录并比较3组患者血清IgA、IgG、IgM、CD4水平及CD4/CD8比值变化情况,以及患儿院内感染率及住院时间.结果3组患儿治疗后的血清IgA、IgG、IgM、CD4水平及CD4/CD8比值水平均高于治疗前(P均<0.05),且L组、H组患儿血清IgA、IgG、IgM、CD4水平及CD4/CD8比值水平均高于C组(P均<0.05),H组患儿血清IgA、IgG、IgM、CD4水平及CD4/CD8比值水平高于C组和L组(P均<0.05),差异均有统计学意义.H组和L组患儿院内感染率、住院时间均低于C组(P均<0.05),L组患儿院内感染率、住院时间均低于C组(P均<0.05).结论使用谷氨酰胺可改善患儿免疫功能的各项指标,增强患儿的体液及细胞免疫功能,减轻病毒对患儿的损害,值得推广应用. Objective To investigate the effect of glutamine on humoral and cellular immune function in children with severe HFMD.Methods 96 children with severe HFMD were enrolled in this study.They were randomly divided into three groups:group C(without glutamine enteral nutrition group,32 cases),group L(low dose 0.2 g/(kg·d)glutamine enteral nutrition group),and group H(high dose of 0.4 g/(kg·d)glutamine enteral nutrition group,32 cases),fasting morning blood on the first day,the eighth day and the fifteenth day of enteral nutrition support,The serum levels of IgA,IgG,IgM,CD4,CD4/CD8,and the nosocomial infection rate and hospital stay time in the three groups were recorded and compared.Results After the treatment,serum levels of IgA,IgG,IgM,CD4 and CD4/CD8 in three groups were significantly higher than those before treatment(all P<0.05),and The levels of serum IgA,IgG,IgM,CD4 and CD4/CD8 in group L and group H were significantly higher than those in group C(all P<0.05),The levels of serum IgA,IgG,IgM,CD4 and CD4/CD8 in group H were significantly higher than those in group C and group L(all P<0.05),all the differences were statistically significant.The infection rate and hospital stay in H group and L group were lower than those in C group(all P<0.05).The infection rate and hospital stay in group L were lower than that in group C(all P<0.05).Conclusion glutamine can improve the immune function of children,which can enhance the humoral and cellular immune function of children,reduce the damage of virus to children,and is worthy of popularization and application.
