摘要
目的通过回顾性分析垂体腺瘤的影像学资料及病理学资料,探讨垂体腺瘤的侵袭性。方法回顾性分析59例垂体腺瘤的影像学资料及病理学资料。结果影像结果:侵袭性垂体腺瘤33例,非侵袭性垂体腺瘤26例。肿瘤>3 cm 20例,均为侵袭性垂体腺瘤;肿瘤<3 cm 39例,其中13例为侵袭性垂体腺瘤,26例为非侵袭性垂体腺瘤。病理结果:PRL腺瘤8例,其中侵袭性腺瘤5例,非侵袭性腺瘤3例;GH腺瘤6例,其中侵袭性腺瘤3例,非侵袭性腺瘤3例;TSH腺瘤2例,均为侵袭性腺瘤;ACTH腺瘤2例,均为非侵袭性腺瘤;混合性腺瘤28例,其中侵袭性腺瘤13例,非侵袭性腺瘤15例;无分泌功能腺瘤13例,其中侵袭性腺瘤10例,非侵袭性腺瘤3例。Ki-67截断值:33例侵袭性腺瘤中23例>3%,10例<3%;26例非侵袭性腺瘤均<3%。结论侵袭性垂体腺瘤的诊断应综合考虑影像学、病理结果及术中所见。垂体腺瘤的侵袭可能与其分泌激素类型无关,发生侵袭的机制仍需进一步深入研究。
Objective The imaging and pathological data of pituitary adenoma were analyzed retrospectively to investigate the invasiveness of pituitary adenoma.Methods The imaging and pathological data of 59 pituitary adenomas were analyzed retrospectively.Results Imaging results:there were 33 cases of invasive pituitary adenoma and 26 cases of non-invasive pituitary adenoma.Tumors larger than 3 cm:20 cases were all invasive pituitary adenomas.Tumor less than 3 cm:39 cases,including 13 invasive pituitary adenomas and 26 non-invasive pituitary adenomas.Pathological results:PRL adenoma:8 cases,including 5 cases of invasive adenoma and 3 cases of non-invasive adenoma.GH adenomas:6 cases,including 3 cases of invasive adenomas and 3 cases of non-invasive adenomas.TSH adenomas:2 cases,all of which were invasive adenomas;ACTH adenoma:2 cases,all non-invasive adenomas;Mixed adenomas:28 cases,including 13 cases of invasive adenomas and 15 cases of non-invasive adenomas.Non-secretory adenomas:13 cases,including 10 cases of invasive adenomas and 3 cases of non-invasive adenomas.Ki-67 cut-off value:among 33 invasive adenomas,23 cases were greater than 3%,and 10 cases were less than 3%.All the 26 cases of non-invasive adenomas were less than 3%.Conclusion Imaging,pathological findings and intraoperative findings should be taken into account in the diagnosis of invasive pituitary adenoma.The invasion of pituitary adenoma may have nothing to do with its secretory hormone type,and the mechanism of the invasion still needs to be further studied.
