摘要
目的分析中性粒细胞/淋巴细胞比值(NLR)及血小板/淋巴细胞比值(PLR)在结缔组织病相关性间质性肺疾病(CTD-ILD)中的临床意义。方法选取2016年1月至2019年2月南通大学第二附属医院风湿免疫科收治的109例CTD-ILD患者作为研究对象,选取同期109名健康体检者作为对照组,比较两组NLR、PLR差异,分析CTD-ILD患者NLP和PLR与各项临床指标的相关性。结果CTD-ILD患者的NLR、PLR明显高于对照组(5.09±0.76比2.15±0.47,243±38比115±26)(P<0.05),丙氨酸转氨酶、天冬氨酸转氨酶、血肌酐无明显变化(P>0.05)。相关性分析显示,NLR、PLR、铁蛋白水平及吸烟史与肺一氧化碳弥散量(D LCO)显著相关(P<0.05),其他指标与D LCO无明显相关性(P>0.05);吸烟的曲线下面积(AUC)为0.504(P=0.17);NLR界值为4.20,AUC为0.776(P=0.03);PLR界值为170,AUC为0.734(P=0.04);铁蛋白界值为510μg/L,AUC为0.689(P=0.06);联合诊断的AUC为0.894(P<0.001)。结论CTD-ILD患者NLR、PLR明显升高,且NLR、PLR升高与肺功能减退有明显相关性。
Objective To evaluate the clinical significance of neutrophils/lymphocyte ratio(NLR)and platelet/lymphocyte ratio(PLR)connective tissue disease associated interstitial lung disease(CTD-ILD)patients.Methods From Jan.2016 to Feb.2019,109 patients with CTD-ILD admitted to the Department of Rheumatology and Immunology of the Second Affiliated Hospital of Nantong University were included in the study,and 109 healthy people during the same period were included as a control group.NLR,PLR were compared between two groups.The correlations between CTD-ILD,NLP,PLR and clinical indexes were analyzed.Results The NLR,PLR of patients with CTD-ILD were significantly higher than those of the control group(5.09±0.76 vs 2.15±0.47,243±38 vs 115±26)(P<0.05),while alanine aminotransferase,aspartate aminotransferase and serum creatinine were not significantly changed(P>0.05).Correlation analysis showed that NLR,PLR,ferritin and smoking history were significantly correlated with diffusion capacity of carbon monoxide in lung(D LCO)(P<0.05),while other indicators were not significantly correlated with D LCO(P>0.05).The area under curve(AUC)of smoking was 0.504(P=0.17);critical value of NLR was 4.20,its AUC was 0.776(P=0.03);critical value of PLR was 170,its AUC was 0.734(P=0.04);critical value of ferritin was 510μg/L,its AUC was 0.689(P=0.06);and the AUC of combined diagnosis was 0.894(P<0.001).Conclusion In CTD-ILD patients,NLR and PLR are significantly increased,and the increase of NLR and PLR is significantly correlated with pulmonary dysfunction.
