摘要
目的分析恶性血液病患者造血干细胞移植前PRA-Ⅰ、Ⅱ类及MICA抗体阳性率和特异性频率。方法应用LSA方法检测恶性血液病患者PRA-Ⅰ、Ⅱ类及MICA抗体,并以已发表文献天然抗体阳性率及特异性频率作为参考,比较分析。结果男性恶性血液病患者抗体阳性率分别为Ⅰ类(56.1%,37/66)、Ⅱ类(45.5%,30/66)、MICA(12.1%,8/66);女性患者阳性率分别为Ⅰ类(77.6%,52/67)、Ⅱ类(53.7%,36/67)、MICA(16.5%,11/67)。恶性血液病患者抗体特异性频率与天然抗体相比明显升高的分别为:A^*23∶01,A^*68∶02,B^*44∶03,B^*57∶01,B^*58∶01,C^*03∶04,DRB1^*16∶01,DRB1^*16∶02,DRB1^*04∶03,DRB1^*04∶01,DQA1^*02∶01,DQB1^*03∶01,DQA1^*03∶02,DQB1^*03∶03,DPA1^*02∶02,DPB1^*05∶01,DPA1^*01∶03和DPB1^*01∶01。结论恶性血液病男性患者PRA-Ⅱ类、MICA抗体阳性率显著高于天然抗体组;女性患者PRA-Ⅰ、Ⅱ类、MICA抗体阳性率均显著高于天然抗体组。从抗体特异性来看,恶性血液病患者既存在天然抗体,也存在非天然抗体,而后者更容易成为DSA。因此,恶性血液病患者在造血干细胞移植前检测PRA-Ⅰ、Ⅱ类和MICA抗体有重要意义。
Objective To investigate the positive rates and specificities of PRA-Ⅰ,Ⅱ,MICA antibodies in patients with hematologic malignancies before hematopoietic stem cell transplantation.Methods The LSA method was applied to detect PRA-Ⅰ,Ⅱand MICA antibodies in patients with hematologic malignancies.Statistical analysis was conducted with the positive rate and specific frequency of natural antibodies from the non-sensitized healthy males published in the literature.Results Positive rates in male patients wereⅠ(56.1%,37/66),Ⅱ(45.5%,30/66)and MICA(12.1%,8/66),those in female patients wereⅠ(77.6%,52/67),Ⅱ(53.7%,36/67)and MICA(16.5%,11/67).The specificities frequencies that significantly elevated in patients compared with the natural antibodies in the controls were A^*23∶01,A^*68∶02,B^*44∶03,B^*57∶01,B^*58∶01,C^*03∶04,DRB1^*16∶01,DRB1^*16∶02,DRB1^*04∶03,DRB1^*04∶01,DQA1^*02∶01,DQB1^*03∶01,DQA1^*03∶02,DQB1^*03∶03,DPA1^*02∶02,DPB1^*05∶01,DPA1^*01∶03 and DPB1^*01∶01.Conclusions The positive rates of PRA-Ⅱ,MICA in male patients and PRA-Ⅰ,Ⅱ,MICA in female patients were significantly increased than the natural antibodies.The patients with hematologic malignancies carried not only the natural antibodies,but also some common specificities that are different from the natural antibodies,and the latter was more likely to become DSA.Therefore,it was of great importance to detect PRA-Ⅰ,Ⅱand MICA antibodies before hematopoietic stem cell transplantation.
