摘要
目的通过血氧水平依赖性-功能性磁共振成像,观察针刺后续效应,探讨不同强度电针刺激健康志愿者上巨虚(ST37)脑功能活动差异。方法将22名健康志愿者随机分为低强度电针组和高强度电针组。2组均针刺左侧上巨虚,辅助针在距离上巨虚0.3~0.5 cm近心端浅刺,电针疏密波,2~50 Hz,脉冲宽度0.2~0.6 ms,低强度电针组电流量以受试者感受穴位有微弱电流刺激为度,高强度电针组以受试者感觉穴位有强烈电流刺激而不痛为度。采集电针刺激前静息态和电针刺激后2个静息态数据,比较不同强度电针刺激前后具有显著差异的脑区及磁共振信号时间域振幅(AM)变化情况。结果不同强度电针刺激前后具有显著差异的脑区:颞上回,颞中回,颞下回,中央前回;顶上小叶,顶下小叶,中央后回;舌回,梭状回,楔叶,距状裂周围皮层;额中回,眶额叶皮层;岛叶,丘脑,前扣带回,后扣带回。与电针刺激前静息态比较,低强度电针组AM大部分减弱,高强度电针组AM大部分增强。结论不同强度电针刺激导致不同的脑功能活动。低强度电针组AM大部分减弱,高强度电针组AM大部分增强。
Objective To observe the sustained effects after acupuncture through blood oxygen level dependent-functional magnetic resonance image(BOLD-fMRI);To explore the differences in brain function activities of Shangjuxu(ST37) on healthy volunteers with different intensities of electroacupuncture. Methods Totally 22 healthy volunteers were randomly divided into low-intensity electroacupuncture stimulation group(L-EAS) and high-intensity electroacupuncture stimulation group(H-EAS). Both groups received electroacupuncture at Shangjuxu(ST37). The auxiliary needle was shallowly punctured at a distance of Shangjuxu(ST37) about 0.3 to 0.5 cm near the heart, electroacupuncture was densely waved, 2 to 50 Hz, and the pulse width was 0.2 to 0.6 ms. L-EAS was stimulated by the weak current of the subjects’ acupoints, and H-EAS was stimulated by the subjects with a strong current without pain. The resting data before and the electroacupuncture stimulation and two resting state data after the electroacupuncture stimulated were collected. The time domain amplitude(AM) of magnetic resonance signals was compared in brain regions with significant differences before and after different intensities of electroacupuncture stimulation. Results The brain regions with significant differences of different intensities of electroacupucture stimulation were as followed: superior temporal cortex, middle temporal cortex, inferior temporal cortex, precentral;superior parietal lobule, inferior parietal lobule, postcentral;lingual gyrus, fusiform gyrus, cunces, calcarine;middle front gyrus, orbitofrontal cortex;nsula, thalamus, anterior cingulated cortex, posterior cingulated cortex. Compared with the resting state before electroacupucture stimulation, the AM in the L-EAS was mostly weakened, and the AM in the H-EAS was mostly enhanced. Conclusion Different intensities of electroacupucture stimulation can induce different brain functional activities. The AM of L-EAS is mostly weakened, and the AM of H-EAS is mostly enhanced.
