阅片列表在耳科住院医师颞骨CT影像阅片教学的应用与实践 被引量:1

摘要 目的探索阅片列表在耳科住院医师颞骨CT影像阅片教学的应用及效果。方法选取2015年1月及2018年12月在中山大学附属第一医院耳科进行住院医师规范化培训88名学员为研究对象,按参加规培时间先后分为对照组和实验组,2015年1月~2016年12月期间42名住院医师为对照组,教学模式为带教过程阅读颞骨CT时没有采用阅片列表;2017年1月~2018年12月期间46名住院医师为实验组,带教模式为混合性教学方式,规培前期没有采用阅片列表,规培中期以后采用阅片列表。每位学员在耳科规培前,规培中期,规培结束进行颞骨CT阅片能力测试,规培结束后均进行问卷调查,比较两组成绩差异及问卷调查评价。结果两组学员在年龄、性别、规培前、规培中期颞骨CT阅片成绩等方面差异均无统计学意义(均P>0.05)。耳科规培结束时颞骨CT阅片考试成绩,实验组学员优于对照组学员,差异具有统计学意义(P<0.05)。实验组学员对在学习兴趣、阅片效率、阅片条理系统性、准确阅片自信心、阅片自主学习能力、手术风险评估能力、医患沟通能力、两种教学模式的满意程度的评价均优于对照组学员,差异均具有统计学意义(P<0.05)。结论利用阅片列表,可培养规范、系统的阅片习惯,有助于提高耳科住院医师学习兴趣,有效提高住院医师手术风险预测能力及颞骨CT阅片能力。 Objective To explore the application and effect of reading list in the teaching of temporal bone CT image reading for otologists.Methods 88 trainees were selected from the Department of Otology,the First Affiliated Hospital of Sun Yat-sen University in January 2015 and December 2018.They were divided into control group and experimental group according to the time of participating in regular training.From January 2015 to December 2016,42 residents served as control group.The teaching mode was that there was no reading list during the teaching process of reading temporal bone CT.During December,46 resident doctors were in the experimental group.The teaching mode was a mixed teaching mode.The reading list was not used in the pre-training period,but after the mid-training period,the reading list was used.Before and during the middle period of otology training,each trainee was tested for the ability of temporal bone CT film reading at the end of training.Questionnaires were conducted after training.The differences of the two groups’ scores and the evaluation of the questionnaire were compared.Results There were no significant differences in age,sex,pre-training and mid-training time between the two groups(P >0.05).At the end of otology training,the scores of temporal bone CT examination in the experimental group were better than those in the control group,and the difference was statistically significant(P<0.05).The students in the experimental group were better than those in the control group in terms of interest in learning,efficiency of film reading,systematic reading,self-confidence in accurate film reading,self-learning ability of film reading,ability of risk assessment of operation,communication ability between doctors and patients,satisfaction and evaluation of the two teaching modes,and the difference was statistically significant(P<0.05).Conclusion Using the reading list can cultivate a standardized and systematic reading habit,help to improve the interest of otologists in learning,and effectively improve th
