摘要 近年来逐渐发现姜黄素具有抗脏器纤维化的潜能,可通过不同途径发挥抗肺纤维化、肝纤维化、心纤维化及肾纤维化作用,在临床治疗中被广泛应用。但姜黄素抗纤维化目前仍有如下问题亟需解决:①姜黄素通过多种生长因子、细胞因子、多条细胞信号通路共同参与、相互作用参与抗纤维化,但具体作用机制尚不明确;②姜黄素有酚性和非酚性成分,各成分抗纤维化机制不明确;③姜黄素抗纤维化作用相关临床试验较少,且临床疗效有待进一步证实;④姜黄素在皮肤、消化、血液、神经等系统中抗炎、抗氧化、抗纤维化和抗肿瘤等药理作用研究不足;⑤姜黄素抗纤维化存在剂量依赖性,临床用药剂量不合理且存在不良反应。因此,今后需进一步加强姜黄素抗纤维化作用机制的研究,为临床治疗提供理论依据和思路;明确姜黄素酚性和非酚性成分抗纤维化的机制及在各脏器中作用机制的差异性;进一步加强大样本临床试验的力度,同时规范姜黄素用药的临床剂量,关注其用药合理性与不良反应。 In recent years,curcumin has been gradually found to have the potential of anti-organ fibrosis. It can play the role of anti-lung fibrosis,anti-liver fibrosis,anti-heart fibrosis and anti-kidney fibrosis through different ways,and is widely used in clinical treatment. However,curcumin still has the following problems to be solved:(1) curcumin is involved in anti fibrosis through multiple growth factors,cytokines and multiple cell signaling pathways,but the specific mechanism is not clear;(2) curcumin has phenolic and non phenolic components,and its anti-fibrosis mechanism is not clear;(3) there are few clinical trials related to the anti-fibrotic effect of curcumin,and the clinical efficacy needs to be further confirmed;(4) the research on the anti-inflammatory,anti-oxidation,anti-fibrotic and anti-tumor pharmacological effects of curcumin in skin,digestion,blood,nervous system and other systems is insufficient;(5) the anti-fibrotic effect of curcumin is dose-dependent,the clinical dosage is unreasonable and there are adverse reactions. Therefore,in the future research,it is necessary to further strengthen the research on the anti-fibrosis mechanism of curcumin to provide theoretical basis and ideas for clinical treatment;clarify the anti fibrosis mechanism of curcumin phenolic and non phenolic components and the differences in the mechanism of action in various organs;further strengthen the strength of large sample clinical trials,standardize the clinical dosage of curcumin,and pay attention to its rationality and adverse reactions.
