冠心病患者脉压及脉压指数与缺血性卒中预后的关系 被引量:6

Relationship between Pulse Pressure and Pulse Pressure Index of Ischemic Stroke in Patients with Coronary Heart Disease

摘要 目的探讨冠心病患者收缩压(SBP)、平均动脉压(MAP)、动态脉压(PP)和动态脉压指数(PPI)与缺血性卒中预后的相关性。方法共入选100例于2011年9月至2013年10月在保定市第四中心医院神经内科进行治疗并进行动态血压监测的缺血性卒中患者,其中50例患者合并有冠心病,对可能影响合并冠心病的脑卒中患者预后的因素经单因素分析初步筛选后,进行多因素Logistic回归分析并通过受试者工作特征(ROC)曲线对其预后进行评估。结果合并有冠心病的缺血性卒中患者预后更差(P<0.01),且预后不良患者的SBP、MAP、PP和PPI同预后良好患者存在统计学差异(P<0.05)。Logistic回归模型分析显示,SBP、MAP和PP与合并有冠心病的缺血性卒中患者的预后相关,而ROC曲线分析结果显示SBP、PP、PPI对缺血性性卒中的预后的评估又优于MAP。结论合并有冠心病的缺血性卒中患者的SBP和PP与其卒中预后密切相关,且对其预后的评价效果优于MAP。 Objective To investigate the correlation between systolic blood pressure(SBP),mean arterial pressure(MAP),dynamic pulse pressure(PP)and dynamic pulse pressure index(PPI)and prognosis of ischemic stroke in patients with coronary heart disease.Methods A total of 100 patients with ischemic stroke who underwent ambulatory blood pressure monitoring in the Department of Neurology of Baoding Fourth Central Hospital from September 2011 to October 2013 were enrolled.Among them,50 patients were complicated with coronary heart disease.The factors that may affect the prognosis of stroke patients with coronary heart disease were preliminarily screened by single factor analysis.Multivariate logistic regression analysis was performed and the prognosis was evaluated by ROC curve.Results The prognosis of ischemic stroke patients with coronary heart disease was worse(P<0.01),and the SBP,MAP,PP and P PI of patients with poor prognosis were significantly different from those of patients with good prognosis(P<0.05).Logistic regression model analysis showed that SBP,MAP and PP were correlated with the prognosis of ischemic stroke patients with coronary heart disease,while ROC curve analysis showed that SBP,PP and PPI were better than MAP in evaluating the prognosis of ischemic stroke.Conclusion SBP and PP in ischemic stroke patients with coronary heart disease are closely related to the prognosis of stroke,and the prognostic evaluation effect is better than that of MAP.
