摘要
目的利用上海市闵行区成年双生子相关调查数据,分析环境以及遗传因素对成年居民肥胖相关指标的影响,为相关疾病防治策略的制定提供更加可靠的依据。方法以上海市闵行区自愿参加调查的成年双生子作为研究对象,在2018年11-12月,通过体型测量、问卷调查、卵型鉴定等方法收集相关信息,运用组内相关系数法计算其肥胖相关指标的遗传度,运用统计学方法对成年双生子的身高、体重、体质指数(BMI)、腰围(WC)、腰高比(WHtR)等肥胖相关指标进行影响因素分析。结果上海市闵行区≥18周岁成年同卵双生子组内相关系数r均高于异卵双生子;男性双生子的身高、体重、BMI、WC、WHtR平均遗传度分别为0.75、0.77、0.78、0.49、0.70,女性双生子分别为0.87、0.79、0.71、0.81、0.80;多因素分析结果显示:女性发生各类肥胖(包括中心性肥胖和腹型肥胖)的危险性均低于男性(OR=0.059、0.145、0.191);体力活动是BMI、WC、WHtR的保护因素(OR=0.046、0.443、0.473),每周至少5天,每天至少持续30 min的中等或高强度体力活动的人发生肥胖的危险性低;食用水果是BMI的保护因素(OR=0.083),每日食用水果的人群发生肥胖的危险性低于不食用水果的人群;年龄是WHtR的影响因素,随着年龄增长,发生腹型肥胖的危险性增加,41~65岁发生肥胖的危险性是18~40岁的2.102倍。结论上海市闵行区成年男性双生子各项肥胖相关指标均受遗传因素影响较大,成年女性双生子各项肥胖相关指标中WC受环境因素影响较大;成年双生子肥胖主要受性别、体力活动、食用水果等因素的影响;中心性肥胖主要受性别、体力活动等因素的影响;腹型肥胖主要受性别、年龄、体力活动等因素的影响。
Objective To analysis the environmental and genetic factors on the obesity related indicators by the data of adult twins in Minhang District of Shanghai, provide more reliable basis for the formulation of related disease prevention and control strategies. Methods The adult twins in Minhang District of Shanghai, who volunteered to participate in the twin investigation,were selected as the research object. From November to December 2018, relevant information was collected through physical examination, questionnaires, egg type identification and other methods. Heritability was calculated by intra-group correlation coefficient method. statistical methods were involved to analyze the height, weight, body mass index(BMI), waist circumference(WC) and waist to height ratio(WHtR) and other obesity related indicators. Results The relative coefficients(r) of monozygotic adult twins over 18 years old in Minhang District of Shanghai were higher than those of the dizygotic twins. The average heritability of height, weight, BMI, WC, WHtR of males was respectively 0.75, 0.77, 0.78, 0.49 and 0.70, while those of werefemales were 0.87, 0.79, 0.71, 0.81 and 0.80 respectively. Multifactor analysis showed that the risk of obesity(including central obesity and abdominal obesity) in women were lower than that in men(OR=0.059, 0.145, 0.191). Physical activity was a protective factor for BMI, WC, WHtR(OR=0.046, 0.443, 0.478). People with moderate or high-intensity physical activity lasting at least 30 minutes a day for at least 5 days a week had a lower risk of obesity. Fruit intake was a protective factor for BMI(OR=0.083), and the risk of obesity was lower in the group with sufficient fruit intake. Age was a risk factor for WHtR. The risk of abdominal obesity increases with age, and the risk of obesity in the age of 41-65 years was 2.102 times higher than in the age group of 18-40 years.
