摘要
目的:基于颈椎骨龄定量分期法(quantitative cervical vertebra maturation,QCVM),探讨正常骨面型舌骨位置在颈椎骨龄不同时期的变化。方法:从门诊选择340例8~18岁在厦门医学院附属口腔医院正畸科就诊的正常骨面型(1°<ANB角<5°)初诊患者,根据不同的颈椎成熟阶段(QCVMⅠ~Ⅳ)分为生长发育高峰前期QCVM-Ⅰ组,高峰期QCVM-Ⅱ组和高峰后期QCVMⅢ~Ⅳ组。所有患者治疗前均拍摄CBCT,在Dolphin软件中截取正中矢状面图像,并对舌骨在垂直向及水平向位置进行定点和角度及距离测量分析。结果:男女舌骨垂直向(S-H)和水平向(H-C3)位置在高峰前期、高峰期、高峰后期差异均有统计学意义(P<0.05)。在同一颈椎骨龄时期,男女垂直向位置(S-H)和水平向位置(H-C3)比较差异均有统计学意义(P<0.05)。结论:正常颌骨面型的舌骨位置在颈椎发育高峰期(QCVM-Ⅱ)向前下方改变最为明显,在正畸临床上此阶段适合舌骨位置的改建;在高峰后期(QCVMⅢ~Ⅳ)男性舌骨位置比女性更为前下。
Objective:To investigate the change of hyoid bone position on cone beam computed tomography(CBCT) images based on quantitative cervical vertebra maturation(QCVM).Methods:Totally 340 patients aged 8~18 years were selected from the outpatient clinic and divided into three groups according to quantitative cervical vertebra maturation(QCVM Ⅰ~Ⅳ)that pre-peak period(QCVM-Ⅰ),peak period(QCVM-Ⅱ)and post-peak period(QCVM Ⅲ~Ⅳ).CBCT was taken before treatment in all patients.The median sagittal images were intercepted in Dolphin software.The hyoid bone was measured in vertical and horizontal directions,and the angle and distance were measured.Results:The vertical and horizontal positions of hyoid bone(S-H and H-C3)were significantly different among QCVM-Ⅰ,QCVM-Ⅱ and QCVM Ⅲ~Ⅳ for both famles and males.There were also significantly different in vertical and horizontal positions(S-H and H-C3)between females and males at the same stage of QCVM.Conclusion:Hyoid bone position changes the most forward and downward during the peak period(QCVM-Ⅱ)in normal skeletal face type for both females and males,which is a good chance to make the hyoid bone movement orthodontically.In contrast to females,however,males move more forward and downward in the post-peak period(QCVM Ⅲ~Ⅳ).
