摘要
目的 探讨上气道二维影像替代三维影像的可能性。方法 1 6名阻塞性睡眠呼吸暂停综合征 ( Obstructive Sleep Apnea Syndrom e,OSAS)患者摄制上气道的磁共振 ( Magnetic Resonance Imag-ing,MRI)片及仰卧位 X线头颅侧位片 ,2 3名 OSAS患者摄制 MRI及直立位 X线片 ,分别对二者两种影像资料中的上气道及软腭、舌体进行比较。结果 上气道阻塞点距腭平面的距离在二者的比较中 ,均无统计学差异。在上气道大小的比较中 ,软腭、舌体的 MRI测量值与 X线测量值相关性较好 ( P<0 .0 5 ) ;仰卧在 X线片在腭咽、舌咽 MRI与 X线指标的二者相关程度较好 ( P<0 .0 5 )直立位则在喉咽的 MRI与 X线符合程度上较好 ( P<0 .0 5 )。但 X线对于阻塞严重程度的分组与 MRI出入较大。结论 在有关上气道的研究中 ,二维影像手段不能完全代替三维影像手段 ;但二维影像能较好地反映软腭、舌体的大小 ,能揭示阻塞的部位 ,能在一定程度上反映腭咽、舌咽、喉咽的大小。仰卧位 X线片优于直立位 X线片。
Objective Can cephalometrics take the place of magnetic resonance imaging(MRI)in diagnosing obstructive sleep apnea syndrome(OSAS)?This article intends to investigate the correspondence and difference in upper airway images between cephalometrics and MRI.Methods There are 16 newly diagnosed OSAS patients who took both MRI and cephalometrics in supine position. And also 23 OSAS patients who took both MRI and cephalometrics in upright position.Compare the site of obstruction,the diameters,section area and the volume of upper airway,and the rank of narrow airway by using MRI with those by using cephalometrics.Results The distance from obstructive site to the palate plane is no significant difference with that measured by MRI.Those measurements in softpalate and tongue show great correspondence between MRI and cephalometrics( P <0.05).Oropharynx in supine position X-ray cephalometrics is correspondent with that in MRI,while hypopharynx in upright position X-ray cephalometrics is correspondent with that in MRI( P <0.05).On the contrast ,the rank narrow airway by using cephalometrics doesn’t coincide with that by MRI.Conclusion The cephalometrics can not be used as substitute of MRI in imaging diagnosis of OSAS.But it has accuracy in revealing the obstructive site and the size of softpalate,tongue and some measurements of pharynx.The supine position is better than upright position.
