摘要
目的:比较不同剂量的阿替普酶治疗老年急性脑梗死患者的临床效果以及对出血风险的影响.方法:选取某院收治的急性脑梗死老年患者130例,采用随机数表法分为对照组和观察组,每组各65例.2组患者都行阿替普酶溶栓治疗,对照组患者使用剂量为0.9mg/kg,观察组患者使用剂量为0.6 mg/kg,采用格拉斯哥昏迷量表(GCS)、美国国立卫生研究院卒中量表(NIHSS)以及改良Rankin量表(mRS)评价2组患者的治疗效果,观察2组患者出血情况.结果:2组患者治疗后GCS、NIHSS、mRS评分与治疗前比较,差异有统计学意义(P<0.05);观察组患者的出血发生率显著低于对照组,差异有统计学意义(P<0.05);2组患者不良反应发生率比较差异无统计学意义(P>0.05).结论:不同剂量阿替普酶治疗老年急性脑梗死患者都有较为明显的临床效果,但小剂量的阿替普酶能够有效降低溶栓治疗后出血的风险,安全性较高,值得推广和使用.
Objective:To compare the clinical effects of different doses of alteplase in the treatment of elderly patients with acute cerebral infarction and the impact on bleeding risk.Methods:130 elderly patients with acute cerebral infarction were randomly divided into a control group and an observation group,with 65 cases in each.Both groups were treated with alteplase thrombolytic therapy.The control group used a dose of 0.9 mg/kg,and the observation group adopted a dose of 0.6 mg/kg.The Glasgow Coma Scale(GCS),the National Institutes of Health Stroke Scale(NIHSS),and the Modified Rankin Scale(mRS)were used to evaluate the effects of the two groups,and their bleeding was observed.Results:There were significant differences in GCS,NIHSS and mRS scores between the two groups before and after the treatment(P<0.05).The incidence of bleeding in the observation group was significantly lower than that in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Different doses of alteplase have significant clinical effects on elderly patients with acute cerebral infarction,but low-dose alteplase can effectively reduce the risk of bleeding after thrombolytic therapy,and it is safe and worthy of promotion and use.
