肌酸激酶 血清肌红蛋白及炎症相关因子的动态监测在多发伤并发多器官功能障碍综合征诊断中的价值 被引量:12

摘要 目的探讨肌酸激酶(CK)、血清肌红蛋白(Mb)及炎症相关因子的动态监测在多发伤合并多器官功能障碍综合征(MODS)诊断中的价值。方法收集我院2013年2月至2017年4月收治的多发伤患者40例,根据MODS诊断标准,分为MODS组和非MODS组,另选择同期我院体检健康者40名(对照组)。监测MODS与非MODS患者不同时间CK、Mb、白细胞介素(IL)-6及肿瘤坏死因子(TNF)-α动态变化,评估各指标间相关关系,以及CK、Mb、IL-6及TNF-α监测对多发伤并发MODS的预测价值。结果MODS患者入院24 h、48 h、72 h时CK、Mb、IL-6和TNF-α均显著高于非MODS患者,差异具有统计学意义(P<0.05);CK、Mb及TNF-α与IL-6间呈现负相关,CK与Mb及TNF-α、Mb与TNF-α均呈现正相关(P<0.05);CK、Mb与IL-6,Mb、IL-6与TNF-α,CK、Mb、IL-6及TNF-α等指标联合检测对多发伤并发MODS具有较高预测价值。结论CK、Mb及炎症相关因子等动态变化均与多发伤患者病情变化密切相关,各指标联合检测对于多发伤并发MODS具有较高的预测价值,值得在临床上推广应用。 Objective To explore the value of dynamic monitoring of creatine kinase(CK),serum myoglobin(MB)and inflammation-related factors in the diagnosis of multiple injuries combined with MODS.Methods Forty cases with multiple trauma in our hospital from February 2013 to April 2017 were selected as subjects,and according to the diagnostic criteria of MODS,patients were divided into MODS group and NON-MODS group,and 40 healthy cases were selected as control group.The levels of serum CK,Mb,IL-6 and TNF-αin patients with MODS and non-MODS at different times were measured,and the correlation between the indicators was evaluated,the predictive value of CK,Mb,IL-6,and TNF-αfor multiple trauma with concurrent MODS was analyzed.Results CK,Mb,IL-6 and TNF-αin patients with MODS were significantly higher than those in non-MODS patients at 24 h,48 h and 72 h(P<0.05).There was a negative correlation between CK,Mb,TNF-αand IL6,and positive correlations between CK and Mb,CK and TNF-α,Mb and TNF-α(P<0.05).The combination detection of CK,Mb and IL-6 has high predictive value for multiple trauma with concurrent MODS,the combination detection of Mb,IL-6 and TNF-αhas high predic-tive value for multiple trauma with concurrent MODS,and the combination detection of CK,Mb,IL-6 and TNF-αhas high predictive value for multiple trauma with concurrent MODS(P<0.05).Conclusion Dynamic changes such as CK,Mb and inflammation-related factors are closely related to the progression of multiple injury patients.The com-bined detection of each index has high predictive value for multiple trauma with MODS.
