摘要
目的探讨CD4^+、CD25^+调节性T细胞(Tregs)和补体C3、C4水平对桥本甲状腺炎(HT)和毒性弥漫性甲状腺肿(Graves病)早期诊断的意义。方法选取河南大学第一附属医院2018年2月-2019年4月30例确诊为HT的患者作为HT组、30例确诊为Graves病的患者作为Graves病组,选取同时期健康体检者30例作为对照组。比较3组CD4^+、CD25^+Tregs/CD4^+T细胞、补体C3、补体C4水平。比较CD4^+CD25^+Tregs/CD4^+T细胞、补体C3、补体C4与三者联合诊断HT和Graves病的ROC曲线及诊断效能。结果 HT组和Graves病组CD4^+CD25^+Tregs/CD4^+T低于对照组(P<0.05);HT组补体C3、补体C4水平低于Graves病组、对照组(P<0.05);CD4^+CD25^+Tregs/CD4^+T细胞、补体C3、补体C4三项指标曲线下面积分别为0.767、0.768、0.736,三者联合检测曲线下面积为0.923,联合检测所得AUC值高于上述三者单独检测(P<0.05)。结论联合检测CD4^+、CD25^+调节性T细胞和补体C3、C4水平可以帮助临床区分HT和Graves病,提高诊断准确率。
Objective To explore significance of CD4^+, CD25^+ regulatory T cells(Tregs) and complement C3, C4 levels for early diagnosis of Hashimoto’s thyroiditis(HT) and toxic diffuse goiter(Graves’ disease). Methods Totally 30 patients who were confirmed with HT in the hospital from February 2018 to April 2019 were enrolled as HT group. 30 patients confirmed with Graves disease were enrolled as Graves disease group. 30 healthy people who underwent physical examination in the same period were enrolled as control group. The levels of CD4^+, CD25^+Tregs/CD4^+ T cells, complement C3 and C4 were compared among the three groups. ROC curves and diagnostic efficiency of CD4^+CD25^+Tregs/CD4^+ T cells, complement C3, complement C4 and their combination for diagnosis of HT and Graves disease were compared. Results CD4^+CD25^+Tregs/CD4^+T in HT group and Graves disease group were lower than that in control group(P<0.05). The levels of complement C3 and C4 in HT group were lower than those in Graves disease group and control group(P<0.05). The area under curves(AUC) of CD4^+ CD5+Tregs/CD4^+ T cells, complement C3 and complement C4 were 0.767, 0.768 and 0.736, respectively. AUC of combined detection with the above indexes was 0.923, which was higher than that of any one of the above three(P<0.05). Conclusion Combined detection of CD4^+, CD25^+ regulatory T cells, and complement C3 and C4 levels can help to differentiate between HT and Graves disease, and improve diagnostic accuracy.
