摘要
目的对比单独应用595 nm脉冲染料激光、单独应用0.5%噻吗洛尔滴眼液及两者联合应用3种不同方案治疗浅表性婴幼儿血管瘤的疗效。方法以2018年6月至2019年6月于山东大学齐鲁儿童医院接受治疗的156例婴幼儿血管瘤患者为研究对象,按照治疗方案的不同分为激光治疗组(n=64)、外敷治疗组(n=60)、联合治疗组(n=32),3组患儿每月接受复查,通过拍照及超声检查记录瘤体变化情况;治疗后随访1个月,记录患儿性别、病变大小、病变部位、治疗次数、治愈率和不良反应。结果联合治疗组的有效率(96.9%,31/32)与激光治疗组(95.3%,61/64)、外敷治疗组(95%,57/60)比较均不存在显著性差异(P>0.05)。与激光治疗组(29.7%,19/64)和外敷治疗组(18.3%,11/60)相比,联合治疗组(56.3%,18/32)中有更多患者疗效评级达到4级(P<0.05),且联合治疗组达到疗效评价4级所需的治疗时间为(3.3±0.21)个月,明显短于激光治疗组、外敷治疗组(P<0.05);3组均无一例出现严重并发症。结论595 nm脉冲染料激光与0.5%噻吗洛尔滴眼液联合应用治疗浅表性婴幼儿血管瘤安全、有效。
Objective To explore the curative efficacy of 595nm pulsed dye laser plus 0.5%timolol for superficial infantile hemangiomas.Methods From June 2018 to June 2019,a total of 156 hemangioma children were recruited and divided into three groups of pulsed dye laser(n=64),external timolol(n=60)and combination treatment(n=32).Three groups were reviewed monthly and tumor changes monitored by photograph and ultrasound.Four levels of efficacy evaluations were assigned and the data of gender,lesion size/location,treatment frequency,curative rate and adverse reactions recorded.The treatment outcomes of three groups were compared.Results The effective rate of combination treatment group(96.9%,31/32)was not statistically different from those of pulsed dye laser group(95.3%,61/64)and timolol group(95%,57/60).As compared with pulsed dye laser(29.7%,19/64)and timolol groups(18.3%,11/60),the efficacy of combination treatment(56.3%,18/32)was more inclined to level 4(P<0.05).The time to reach grade 4 treatment in combination treatment group was(3.3±0.21)months and it was significantly lower than that in either treatment group alone(P<0.05).No serious complications occurred in three groups.Conclusion Curative efficacy of timolol plus 595nm pulsed dye laser is superior to that of simple application of timolol or 595nm pulsed dye laser alone.
