摘要
目的探讨养阴清热方联合含铂类化疗方案治疗在晚期非小细胞肺癌(NSCLC)的临床疗效。方法按照随机数字表法将80例晚期NSCLC患者分为观察组和对照组各40例。对照组采用含铂类化疗方案治疗,观察组在含铂类化疗方案基础上联合养阴清热方治疗。比较两组患者的临床疗效,治疗前后的生存质量、中医证候积分、血清肿瘤标志物[癌胚抗原(CEA)、糖类抗原125(CA125)]表达水平及治疗期间的不良反应。结果观察组患者的客观缓解率(ORR)和疾病控制率(DCR)均高于对照组患者(P﹤0.05);治疗后,观察组患者的卡氏评分(KPS)高于对照组患者而中医证候积分低于对照组患者(P﹤0.05),血清CEA和CA125表达水平均低于对照组患者(P﹤0.05);治疗期间,观察组患者的胃肠道反应、白细胞降低和血小板降低不良反应发生率均低于对照组患者(P﹤0.05),两组患者的肝肾功能异常、心功能异常和神经损伤的发生率比较,差异均无统计学意义(P﹥0.05)。结论养阴清热方联合含铂类化疗方案治疗晚期NSCLC能够提高临床疗效,改善患者的生存质量,降低血清肿瘤标志物表达水平,降低不良反应发生率。
Objective To investigate the clinical efficacy on treatment of advanced non-small cell lung cancer(NSCLC)with Yangyinqingre formula combined with platinum-containing chemotherapy regimens.Method According to the method of random number table,80 patients with advanced NSCLC were divided into the observation group and the control group,40 cases each.The control group was treated with platinum-containing chemotherapy regimen,and the observation group was treated with Yangyinqingre formula on the basis of platinum-containing chemotherapy regimen.The clinical efficacy,quality of life and traditional Chinese medical(TCM)syndrome score,expression level of serum tumor markers[carcino-embryonic antigen(CEA),carbohydrate antigen 125(CA125)]before and after treatment,and adverse reactions during treatment were compared between the two groups.Result The objective response rate(ORR)and disease control rate(DCR)of the observation group were significantly higher than those of the control group(P<0.05).After treatment,the Karnofsky performance status(KPS)in the observation group was significantly higher than this in the control group,while the TCM syndrome score was significantly lower than this in the control group(P<0.05).The levels of CEA and CA125 in serum were significantly lower than those in the control group(P<0.05).During the treatment,the incidence of gastrointestinal reaction,leucopenia and thrombocytopenia in the observation group was lower than that in the control group(P<0.05).There was no significant difference in the incidence of liver and kidney function abnormality,heart function abnormality and nerve injury between the two groups(P>0.05).Conclusion Yangyinqingre formula combined with platinum-containing chemotherapy can improve the clinical efficacy,improve the quality of life of patients,reduce the expression level of serum tumor markers,and reduce the incidence of adverse reactions.
