摘要 目的 探讨冠心病患者经桡动脉行冠状动脉介入(TRA-PCI)术后不同减压时间的临床效果.方法 选取2017年12月—2019年2月间我院心内科60例TRA-PCI术后应用TB Band桡动脉介入穿刺点止血的冠心病患者,随机分成对照组和观察组各30例.对照组于术后4h行首次减压,每隔2 h后再次减压,共3次,每次减压放气量2 mL,术后12 h内放完余气.观察组于术后2 h行首次减压,隔1 h后再次减压,共3次,每次减压放气量2 mL,术后10 h内放完余气.比较两组术后2 h、4 h舒适度评价、压迫处局部并发症和压迫侧食指经皮血氧饱和度(SPO2)监测结果及总体时间内的止血效果.结果 术后减压总体时间内两组患者止血效果差异无统计学意义(P>0.05);术后2 h,两组患者舒适度评价差异无统计学意义,SPO2监测结果均在正常范围,差异无统计学意义(P>0.05);术后4 h,两组患者舒适度评价及SPO2监测结果有明显差异,观察组舒适度评价得分及SPO2监测结果明显优于对照组;术后减压总体时间内,观察组患者压迫处局部并发症发生率明显低于对照组,差异有统计学意义(P<0.05).结论 冠心病患者TRA-PCI首次减压时间提前到术后2 h,解除压迫止血时间总体缩短为10 h,止血效果相当,但患者舒适度评价明显提高,压迫处局部并发症发生率明显降低. Objective To investigate the clinical effects of different decompression time after coronary artery intervention(TRA-PCI)in patients with coronary heart disease.Methods From December 2017 to February 2019,60 patients with coronary heart disease who underwent TRA-PCI after TRA-PCI in our hospital were randomly divided into control group and observation group.The control group underwent the first decompression at 4 hours after surgery,and then decompressed again every 2 hours,a total of 3 times,Each time the decompressed deflation volume was 2 ml,the remaining gas was released within 12 hours after surgery.The observation group was decompressed for the first time 2 hours after the operation,and then decompressed again after 1 hour,a total of 3 times,Each time the decompressed deflation volume was 2 ml,the remaining gas was released within 10 hours after the operation.The comfort evaluation at 2 h and 4 h,the local complications at compression,and the percutaneous oxygen saturation(SPO2)of the compression side index were compared between the two groups.Results There was no significant difference in hemostasis between the two groups(P>0.05).There was no significant difference in comfort evaluation between the two groups.The SPO2 monitoring results were in the normal range,and there was no statistical significance(P>0.05).4 hours after operation,the comfort evaluation and SPO2 monitoring results of the two groups were significantly different.The observation group's comfort evaluation score and SPO2 monitoring results were significantly better than the control group;the postoperative decompression time,the observation group patients'compression The incidence of local complications was significantly lower than that of the control group,and the difference was statistically significant(P<0.05).Conclusion In the patients with coronary heart disease,the first decompression time of TRA-PCI was advanced to 2 h after surgery,and the time to release the hemostasis was shortened to 10 h.The hemostasis effect was equivalent,but t
