摘要
目的:观察冰硼散加减外用联合穴位注射治疗原发性肝癌中重度疼痛的临床疗效,以期为临床治疗原发性肝癌中重度疼痛提供有效途径。方法:将81例原发性肝癌中重度疼痛患者根据随机数字表法分为治疗组41例和对照组40例。对照组给予癌痛三阶梯治疗。治疗组在对照组基础上给予冰硼散外用联合苦参注射液穴位注射。连续治疗14 d,观察两组功能状态评分(Karnofsky,KPS)、疼痛数字评分法(numerical rating scale,NRS)、抑郁自评量表(self-rating depression scale,SDS)、焦虑自评量表(self-rating anxiety scale,SAS)、奥施康定使用剂量、血清炎性因子(前列腺素E2、β-内啡肽、缓激肽)、不良反应等。结果:①两组治疗后完全缓解率为45.1%,有效率为93.5%;对照组完全缓解率为30.0%,有效率为83.3%。两组比较,差异具有统计学意义(P<0.05)。②两组治疗前后KPS、SAS、SDS、NRS、奥施康定使用量比较,差异具有统计学意义(P<0.05),且治疗组优于对照组(P<0.05)。③两组治疗前后前列腺素E2、β-内啡肽、缓激肽水平比较,差异具有统计学意义(P<0.05),且治疗组优于对照组(P<0.05)。④两组均未出现不良反应。结论:冰硼散加减外用联合穴位注射治疗原发性肝癌中重度疼痛确有良好疗效。
Objective:To observe the clinical observation of the treatment of severe hepatocellular carcinoma with primary acupoint injection combined with external application of Bingpeng Powder and external acupoint injection,in order to provide an effective treatment for severe pain in primary liver cancer.Methods:81 cases of severe hepatocellular carcinoma with severe pain were divided into treatment group(41 cases and control group) according to the random number table method.The control group was given the three-step treatment of cancer pain for OxyContin titration and treatment.The treatment group was in the control group.On the basis of group,acupoint injection of external application of Bingpeng Powder and Kushen Injection was given for 14 days.The functional status scores(Karnofsky,KPS),numerical rating scale(NRS) and self-rating depression scale were observed.Self-rating depression scale(SDS),self-rating anxiety scale(SAS),dose of OxyContin,serum inflammatory factors(prostaglandin E2,β-endorphin,bradykinin),adverse reactions,etc.Results:The complete remission rate was 45.1% and the effective rate was 93.5%.The complete remission rate was 30.0% and the effective rate was 83.3%.The two groups were statistically significant in the overall remission rate.(P<0.05);2 K groups and SAS,SDS,NRS,and OxyContin before and after treatment in the two groups were compared.The difference between the two groups was statistically significant(P<0.05).The levels of prostaglandin E2,β-endorphin and bradykinin were statistically significant(P<0.05).There were no adverse reactions in the treatment group.Conclusion:The combination of Bingpeng Powder and external acupoint injection for the treatment of severe pain in primary liver cancer has a good effect.
