摘要
目的探讨应用再生医疗技术联合自体皮在乳腺癌改良根治术后皮瓣坏死修复的临床效果研究。方法回顾性分析我院普外科2015年12月~2018年8月对19例乳腺癌改良根治术后皮瓣坏死患者,采用再生医疗技术结合自体全厚皮片移植修复,皮瓣坏死清创后缺损面积为9cm×12cm^10cm×15cm,创面彻底清创止血后,局部涂抹湿润烧伤膏,同时裁剪与创面合适大小的自体全厚皮片移植,按照常规植皮术操作。结果本组19例应用再生医疗技术联合自体皮修复乳腺癌改良根治术后皮瓣坏死,软组织缺损创面后,术后缺损创面均得到良好覆盖,植皮全部成活,植皮区皮肤弹性好,无渗液、不愈合情况发生,经过正规功能锻炼,患乳同侧关节活动良好,供皮区未见瘢痕形成,术后随访3~6个月。结论采用再生医疗技术联合自体皮移植在乳腺癌改良根治术后皮瓣坏死修复缺损创面,手术操作简单,疗效确切,值得临床应用。
Objective To explore clinical curative effects of regenerative medical technique combined with selfskin grafting in skin flap necrosis repair after modified radical mastectomy for breast cancer.Methods 19 patients with skin flap necrosis after modified radical mastectomy for breast cancer in department of general surgery in our hospital from December 2015 to August 2018 were retrospectively analyzed.They were given regenerative medical technique combined with full thickness skin autograft repair.The defect area of the flap after necrotic debridement was 9cm×12cm to 10cm×15cm.After the wound was thoroughly debrided and hemostasis,the moist burn ointment was applied locally.At the same time,the full thickness skin autograft of the appropriate size of the wound was cut and operated according to the conventional skin grafting operation.Results 19 patients with skin flap necrosis underwent regenerative medical technique combined with self-skin grafting for breast cancer after modified radical mastectomy.After the soft tissue defect wound,the wounds were well covered,the skin grafts all survived,the skin elasticity of the skin graft area was good and no liquid leakage or non-healing occurred.After regular functional exercise,the ipsilateral joint of the affected breast had good movement and no scar formation was observed in the donor skin area.The patients were followed up for 3 months to 6 months after operation.Conclusion Regenerative medical technique combined with self-skin grafting in skin flap necrosis repair of defective wounds after modified radical mastectomy for breast cancer has simple operation and accurate curative effects.It is worthy of clinical application.
