摘要
目的分析我院门急诊糖皮质激素类药物的使用情况,促进临床合理用药。方法抽取2018年1~12月我院门急诊1200份糖皮质激素处方,根据《糖皮质激素类药物临床应用指导原则》《北京市医疗机构处方专项点评指南(试行)》,填写处方点评工作表,进行汇总、分析和点评。结果1200份糖皮质激素处方中,使用频次最多的药物是布地奈德混悬液(35.67%),其次为地塞米松磷酸钠注射液(25.08%)。使用糖皮质激素前5位的科室中,儿科使用率居所有科室的首位(41.17%)。合理处方1018份(84.83%),不合理处方182份(15.17%),主要表现为适应证不适宜(31.87%)和全身性给药不适宜(30.77%),有96份(8.00%)处方难以点评。结论我院糖皮质激素类药物存在不合理应用现象,临床医生应强化用药意识,药师应提高处方审核能力,确保糖皮质激素类药物合理应用。
Objective To analyze the use of glucocorticoids in outpatient and emergency departments of our hospital and promote rational drug use in clinic.Methods A total of 1200 prescriptions of glucocorticoids in outpatient and emergency departments of our hospital from January to December 2018 were selected,according to the Guiding Principles for Clinical Application of Glucocorticoids and the Guidelines for Special Evaluation of Prescriptions in Beijing Medical Institutions(Trial Implementation),the prescription review worksheet was filled in,summarized,analyzed and commented.Results Among 1200 glucocorticoid prescriptions,Budesonide Suspension(35.67%)was the most frequently used drug,followed by Dexamethasone Sodium Phosphate Injection(25.08%).Among the top five departments used glucocorticoids,the paediatric utilization rate ranked first in all departments(41.17%).There were 1018 rational prescriptions(84.83%)and 182 unreasonable prescriptions(15.17%).The main manifestations were inappropriate indications(31.87%)and inappropriate systemic administration(30.77%).96(8.00%)prescriptions were difficult to comment.Conclusion There is unreasonable use of glucocorticoids in our hospital.Clinicians should strengthen the awareness of drug use.Pharmacists should improve the ability of prescription review to ensure the rational use of glucocorticoids.
