摘要
目的通过分析妊娠期糖尿病(gestational diabetes mellitus,GDM)患者妊娠早期血脂代谢状态,为进一步指导GDM患者血糖、血脂水平监测、保健与治疗提供客观依据。方法随机选取2018年1月至2018年8月首都医科大学附属北京世纪坛医院GDM患者110例,正常孕妇116例,记录年龄、孕前体质指数(body mass index,BMI)等信息。回顾性分析GDM患者和正常孕妇妊娠OGTT空腹血糖(fasting blood glucose,FBG)、糖化血红蛋白(glycosylated hemoglobin,HbA1C%)水平,以及妊娠早期总胆固醇(cholesterol,CHLO)、三酰甘油(triglyceride,TRIG)、高密度脂蛋白胆固醇(high density lipoprotein cholesterol,HDL-C)和低密度脂蛋白胆固醇(low density lipoprotein cholesterol,LDL-C)各指标水平。结果GDM患者CHOL、TRIG和LDL-C水平均值均高于对照组组,HDL-C指标水平低于正常对照组,两组间TRIG和HDL-C组间差异具有统计学意义。将GDM患者根据空腹血糖(FBG)和糖化血红蛋白(HbA1C%)分组后,TRIG水平在组间差异具有统计学意义。结论妊娠早期血脂代谢状态尤其是TRIG水平可作为GDM患者早期筛查和预防指标。
Objective To analyze the metabolic status of blood lipids in patients with gestational diabetes mellitus(GDM)in early pregnancy,and to provide an objective knowledge basis for monitoring blood glucose and lipid levels,as well as health care and treatment of GDM patients.Methods From January,2018 to August,2018,110 patients with GDM and 116 normal pregnant women were randomly selected for the study from Beijing Shijitan Hospital affiliated to Capital Medical University.Age,pre-pregnancy body mass index(BMI)and other information were recorded among patients.The levels of fasting blood glucose(FBG),glycosylated hemoglobin(HbA1C%)and total cholesterol(CHLO),triglyceride(TRIG),high-density lipoprotein cholesterol(HDL-C)and low-density lipoprotein cholesterol(LDL-C)in GDM patients and normal pregnant women were analyzed retrospectively.Results The mean levels of CHOL,TRIG and LDL-C in patients with GDM were higher than those in the control group,and the level of HDL-C was lower than that in the normal control group.There were significant differences between the TRIG and HDL-C groups.After grouping GDM patients according to fasting blood glucose(FBG)and glycosylated hemoglobin(HbA1C%),the TRIG levels were significantly different among the groups.Conclusion The metabolic status of blood lipid in early pregnancy,especially the level of TRIG,can be used as an early screening and prevention index for patients with GDM.
