摘要
目的探讨体位管理与助产责任制护理协同干预初产妇对分娩方式和妊娠结局的影响。方法回顾性分析2018年2月至2019年6月于我院分娩的初产妇(85例)的临床资料,视其临床护理方案的不同给予分组,分别为对照组(助产轮班制护理,42例)与观察组(体位管理+助产责任制护理,43例)。观察两组入选者分娩方式,同时记录两组入选者各产程时间及产后2 h出血量,并对比两组入选者妊娠结局。结果干预后,与对照组自然分娩率(69.05%)、剖宫产率(30.95%)相比,观察组自然分娩率(88.37%)更高,剖宫产率(11.63%)更低,差异有统计学意义(P<0.05);与对照组相比,观察组的各产程时间均更短,产后2 h出血量更少,差异有统计学意义(P<0.05);干预后,与对照组产妇不良结局发生率(23.81%)、新生儿不良结局发生率(19.05%)相比,观察组产妇不良结局发生率(6.98%)、新生儿不良结局发生率(2.33%)均更低,差异有统计学意义(P<0.05)。结论体位管理与助产责任制护理协同干预初产妇效果良好,可有效提高产妇自然分娩率,缩短各产程时间,减少产后2 h出血量,改善妊娠结局。
Objective To explore the effect of coordinated intervention of posture management and midwifery responsibility nursing care on delivery methods and pregnancy outcomes.Methods The clinical data of 85 primiparas(85 cases)delivered in our hospital from February 2018 to June 2019 were analyzed retrospectively.According to the different clinical nursing plans,they were divided into control group(42 cases of midwifery shift nursing)and observation group(43 cases of position management+midwifery responsibility nursing).The delivery mode and pregnancy outcome of the two groups were observed.Results After the intervention,compared with the control group's natural delivery rate(69.05%)and cesarean section rate(30.95%),the observation group's natural delivery rate(88.37%)was higher,and the cesarean section rate(11.63%)was lower.There was statistical significance(P<0.05);compared with the control group,the observation group had shorter periods of labor and less blood loss at 2 h postpartum,and the difference was statistically significant(P<0.05);after intervention,compared with the control group compared with the incidence of adverse maternal outcomes(23.81%)and adverse neonatal outcomes(19.05%)in the observation group,the incidence of adverse maternal outcomes(6.98%)and adverse neonatal outcomes(2.33%)in the observation group were lower.The difference was statistically significant(P<0.05).Conclusion The coordinated intervention of body position management and midwifery responsibility system can effectively improve the natural delivery rate and pregnancy outcome of primipara.
