摘要
目的比较两种不同手术方法治疗对老年前列腺增生(BPH)合并膀胱结石患者性功能的影响。方法选取我院2017年1月至2018年9月收治的120例老年BPH合并膀胱结石患者,随机分为观察组和对照组各60例。对照组行经尿道前列腺电切术(TURP)联合气压弹道碎石治疗,观察组行经尿道前列腺双极等离子电切术(PKRP)联合钬激光碎石术治疗。比较两组的手术情况及性功能。结果观察组的术中出血量、膀胱冲洗时间、导尿管留置时间、住院时间均明显优于对照组(P<0.01)。术后,观察组的IIEF-5评分明显高于对照组,勃起功能障碍率明显低于对照组(P均<0.05)。结论PKRP联合钬激光碎石术治疗老年BPH合并膀胱结石患者,对其性功能影响较小,患者术后勃起功能障碍率较低,值得临床推广。
Objective To compare the influence of two different surgical methods on sexual function of elderly patients with benign prostatic hyperplasia(BPH)complicated with bladder stone.Methods 120 cases of elderly patients with BPH complicated with bladder stone admitted to our hospital from January 2017 to September 2018 were selected and randomly divided into observation group and control group,with 60 cases in each group.The control group underwent transurethral resection of the prostate(TURP)combined with pneumatic lithotripsy.The observation group underwent transurethral plasmakinetic resection of prostate(PKRP)combined with holmium laser lithotripsy.The surgical conditions and sexual function of the two groups were compared.Results The intraoperative blood loss,bladder irrigation time,catheter indwelling time and hospitalization time of the observation group were significantly better than those of the control group(P<0.01).After operation,the IIEF-5 score of the observation group was significantly higher than that of the control group,and the erectile dysfunction rate was significantly lower than that of the control group(both P<0.05).Conclusions PKRP combined with holmium laser lithotripsy in the treatment of elderly patients with BPH complicated with bladder stone has little influence on the sexual function,with lower postoperative erectile dysfunction rate,which is worthy of clinical promotion.
