PD-1/PD-L1和CD8^+Tils在宫颈鳞癌组织中的表达及临床意义 被引量:2

Expression and clinical significance of PD-1/PD-L1 and CD8^+Tils in cervical squamous cell carcinoma

摘要 目的探讨PD-1/PD-L1表达及PD-L1结合CD8^+肿瘤浸润淋巴细胞(Tils)密度与宫颈鳞癌患者临床病理特征及预后的关系。方法采用免疫组化染色法检测2008年至2017年山西医科大学第一医院病理科手术切除或活检的131例宫颈鳞癌组织中PD-1/PD-L1表达和CD8^+Tils密度,分析宫颈鳞癌组织中PD-1、PD-L1表达与临床病理特征的关系及PD-L1的表达与CD8^+Tils密度相结合对宫颈鳞癌患者预后的影响。Kaplan-miere法绘制生存曲线,并行Log-rank检验。Cox风险比例回归模型分析影响预后的因素。结果131例宫颈鳞癌患者中PD-L1的阳性率为64.89%,与间质浸润深度、FIGO分期、分化程度、淋巴血管间隙侵犯、淋巴结转移及宫旁浸润有统计学意义(P<0.05);PD-1的阳性率为61.07%;PD-1/PD-L1的表达与CD8^+Tils密度呈显著相关;PD-L1/CD8^+Tils密度组与肿瘤大小、FIGO分期及淋巴血管间隙侵犯有相关性(P<0.05)。PD-L1阴性表达患者较PD-L1阳性表达患者预后好(P<0.05);CD8^+Tils高密度表达患者较CD8^+Tils低密度表达患者预后好(P<0.05);PD-L1-/CD8^+Tils高密度组预后最好,PD-L1^+/CD8^+Tils低密度组预后最差(P<0.05);而PD-1的表达与预后无统计学意义(P>0.05)。Cox多因素分析显示:分化程度、淋巴血管间隙侵犯、淋巴结转移和FIGO分期均为影响宫颈鳞癌患者OS的独立因素。结论宫颈鳞癌组织中PD-1/PD-L1的表达与CD8^+Tils细胞的密度有关。PD-L1结合CD8^+Tils密度可以评估宫颈鳞癌患者的预后,为临床提供新的治疗靶点。 Objective To investigate the relationship between the expression of PD-1/PD-L1 and PD-L1 combined with the density of CD8^+tumor infiltrating lymphocytes(Tils)and the clinicopathological parameters and prognosis in patients with cervical squamous cell carcinoma(SCC).Methods The expression of PD-1/PD-L1 and the density of CD8^+Tils in 131 cases of cervical squamous cell carcinoma were detected by immunohistochemical staining from 2008 to 2017 in the Department of Pathology,First Hospital of Shanxi Medical University.The relationship between the expression of PD-1 and PD-L1 and clinicopathological features in cervical squamous cell carcinoma and the effect of the combination of PD-L1 and CD8^+Tils density on the prognosis of patients with cervical squamous cell carcinoma was analyzed.Kaplan-miere method was used to draw survival curve and parallel Log-rank test.Cox proportional regression model was used to analyze the prognostic factors.Results The positive rate of PD-L1 was 64.89%.The positive rate of PD-L1 was significantly correlated with the depth of interstitial invasion,FIGO stage,differentiation degree,lymphatic space invasion,lymph node metastasis and parauterine invasion(P<0.05).The positive rate of PD-1 was 61.07%and the expression of PD-L1 was significantly correlated with the density of CD8^+Tils.The PD-L1/CD8^+Tils density group was associated with tumor size,FIGO stage and lymphatic space invasion(P<0.05).Patients with negative PD-L1 expression had a better prognosis than those with PD-L1 positive expression(P<0.05).The prognosis of patients with high density expression of CD8^+Tils was better than that of patients with low density expression of CD8^+Tils(P<0.05).The prognosis of PD-L1-/CD8^+Tils high density group was the best,the prognosis of PD-L1^+/CD8^+Tils low density group was the worst(P<0.05).There was no significant difference in PD-1 expression and prognosis(P>0.05).Multivariate analysis showed that the degree of differentiation,lymphatic space invasion,lymph node metastasis and FIGO stagin
