摘要
目的探讨依达拉奉联合阿替普酶对老年急性脑梗死(acute cerebral infarction,ACI)患者脑血流及神经损伤的影响。方法随机将105例老年ACI患者分为常规治疗组(n=52)和联合治疗组(n=53)。常规治疗组采用阿替普酶治疗,联合治疗组在常规治疗组基础上采用依达拉奉治疗。采用经颅多普勒超声检查测定双侧大脑中动脉的平均流速(Vm)和峰流速(Vp)水平。参照美国国立卫生研究院卒中量表(the national institutes of health stroke scale,NIHSS)、简易健康状况量表(short form-36,SF-36)标准评价神经损伤程度和生活质量。结果经治疗后,联合治疗组治疗总有效率较常规治疗组明显增高,Vm、Vp、SF-36评分较治疗前和常规治疗组相应值明显升高,NIHSS评分较治疗前和常规治疗组相应值明显降低(P<0.05)。2组药物不良反应率比较差异无统计学意义(P>0.05)。结论依达拉奉联合阿替普酶可有效安全应用于老年ACI患者的治疗期间,可明显改善脑血流动力学,降低神经损伤程度,值得临床推广应用。
Objective To investigate the effects of edaravone combined with alteplase on cerebral blood flow and nerve injury in elderly patients with acute cerebral infarction(ACI).Methods A total of 105 elderly patients with ACI were randomly divided into conventional treatment group(n=52)and combination treatment group(n=52).The patients in conventional treatment group were treated by alteplase,however,the patients in combination treatment group,on the basis of conventional treatment group,were treated by edaravone.The mean velocity(Vm)and peak velocity(Vp)of bilateral middle cerebral artery were examined by transcranial doppler ultrasonography,and the nerve injury severity and life quality of patients were evaluated according to the national institutes of health stroke scale(NIHSS)and short form-36(SF-36).Results After the treatment,the total effective rate in combination treatment group was significantly higher than that in conventional treatment group,and the Vm,Vp and SF-36 scores were significantly higher than those before treatment and those in conventional treatment group,however NIHSS scores were significantly lower than those before treatment and those in conventional treatment group(P<0.05).Moreover there was no significant difference in the incidence rate of adverse drug reaction between the two groups(P>0.05).Conclusion The edaravone combined with alteplase is safe and effective in treating ACI in elderly patients,which can significantly improve cerebral hemodynamics,reduce nerve injury severity,therefor,it is worth using widely in clinical practice.
