摘要
目的探讨多学科合作团队康复模式在急性缺血性脑卒中(AIS)抑郁患者中的应用价值。方法郑州市第七人民医院收治的162例AIS抑郁患者,选取2017-01—2018-06纳入的81例AIS抑郁患者作为对照组(常规方法进行干预),2018-07—2019-01纳入的81例患者作为观察组(在对照组基础上联合多学科合作团队康复模式的干预),干预前后汉密尔顿抑郁量表(HAMD)评估2组心理状况,比较2组干预效果,日常生活活动能力评估量表(ADL)以及美国国立卫生研究院卒中量表(NIHSS)评估2组患者的预后,同时观察2组并发症发生情况。结果与对照组比较,干预后1、2、3个月观察组HAMD评分显著低,而观察组责任护士方面、健康指导、心理指导、病区环境方面评分也显著高于对照组(P<0.05),干预后与对照组相比较,观察组ADL评分明显高,NIHSS评分明显低,并且观察组并发症发生率显著低,差异有统计学意义(均P<0.05)。结论多学科合作团队康复模式干预应用于AIS抑郁患者中,在明显减轻患者抑郁情绪、提高干预效果、改善预后及减少并发症发生方面有积极作用。
Objective To explore the application value of multidisciplinary team rehabilitation pattern in patients with depression after acute ischemic stroke(AIS).Methods A total of 162 patients with depression after AIS in the hospital were selected as the subjects.81 of them admitted between January 2017 and June 2018 were included in the control group(routine intervention)while the other 81 patients admitted between July 2018 and AIS 2019 were included in the observation group(intervention of multidisciplinary team rehabilitation pattern on the basis of the control group).The psychological status of both groups was evaluated with the Hamilton Depression Scale(HAMD)before and after the intervention.The intervention effects were compared between the two groups,and the prognosis was evaluated with the Activity of Daily Living scale(ADL)and the National Institutes of Health Stroke Scale(NIHSS).The occurrence of complications in both groups was observed.Results HAMD scores of the observation group were significantly lower than those of the control group while scores of responsible nurses,health guidance,psychological guidance and ward environment were significantly higher than those of the control group at 1 month,2 months and 3 months after intervention(P<0.05).The ADL score of the observation group was significantly higher than that of the control group while the NIHSS score was significantly lower than that of the control group after intervention.Besides,the incidence of complications was significantly lower in the observation group(all P<0.05).Conclusion The application of multidisciplinary team rehabilitation pattern in patients with depression after AIS plays a positive role in relieving depression,improving intervention effects and prognosis and reducing occurrence of complications.
