摘要
目的:探究彩色多普勒超声对颈动脉斑块形态及其稳定性的诊断价值。方法:选取医院收治的126例脑梗死患者,采用MVU-8102型多功能血管超声仪检测其颈动脉斑块,并观察其稳定性及不同斑块分布情况,以数字减影血管造影(DSA)检查结果为金标准,分析彩色多普勒超声对颈动脉斑块形态及其稳定性的诊断价值。结果:在126例患者中经彩色多普勒超声共检出286处颈动脉斑块,DSA检出308处块斑块,彩色多普勒超声对颈动脉内膜中层厚度(IMT)增厚、稳定斑块、易损斑块、正常检出率与DSA相较,差异无统计学意义(x^2=0.002,x^2=0.060,x^2=0.000,x^2=0.287;P>0.05),总检出率为92.86%;彩色多普勒超声检出颈总动脉(CCA)斑块数量明显高于颈内动脉(ICA)和颈外动脉(ECA)斑块数量,与DSA检出结果基本相符,差异无统计学意义(x^2=2.782,x^2=3.756,x^2=0.011;P>0.05)。结论:彩色多普勒超声可有效检出颈动脉斑块形态以及斑块稳定性,可作为脑血管疾病患者预测病情严重程度的有效检查手段。
Objective:To investigate the diagnostic value of color Doppler ultrasound for morphology and stability of carotid plaques.Methods:A total of 126 patients with cerebral infarction who received treatment in hospital were enrolled.Multifunctional vascular ultrasound instrument of MVU-8102 type was performed to detect carotid plaques so as to observe the stability and distribution of different plaques.The results of digital subtraction angiography(DSA)were used as the gold standard to analyze the diagnostic value of color Doppler ultrasound for morphology and stability of carotid plaques.Results:In the 126 patients,a total of 286 carotid plaques were detected by color Doppler ultrasound,and 308 plaques were detected by digital subtraction angiography(DSA).There were no significant differences in the detection rates of the thickening of intima-media thickness(IMT)of carotid artery,stabilized plaque,vulnerable plaque and normal between color Doppler ultrasound and DSA(x^2=0.002,x^2=0.060,x^2=0.000,x^2=0.287,P>0.05).And the total detection rate was 92.86%.As the detected results of color Doppler ultrasound,the number of the plaques of common carotid artery(CCA)was significantly higher than that of internal carotid artery(ICA)and external carotid artery(ECA),respectively,which was roughly consistent with that of DSA.And the differences of them between color Doppler ultrasound and DSA were no significant(x^2=2.782,x^2=3.756,x^2=0.011,P>0.05).Conclusion:Color Doppler ultrasound can effectively detect morphology and stability of carotid plaques,and it can be used as an effective means to predict the severity of disease for patients with cerebrovascular disease.
