摘要
目的观察血府逐瘀汤联合盐酸西替利嗪片对血瘀型荨麻疹患者血清中细胞因子及T淋巴细胞水平的影响。方法将100例符合纳入标准的血瘀型荨麻疹患者分为对照组和研究组,每组各50例,对照组给予口服盐酸西替利嗪片,研究组在对照组基础上联合血府逐瘀汤治疗;比较两组患者瘙痒程度、风团大小、皮损分布、皮红斑程度、皮损数量等变化情况及两组临床疗效,并分析比较两组患者治疗前后血清中的细胞因子,如白细胞介素4(IL-4)、IL-10、IL-17、IL-23、IL-35及T淋巴细胞(CD3+、CD4+、CD8+、CD4+/CD8+)水平的变化。结果研究组患者治疗前后风团大小、风团数量、皮损分布、瘙痒程度、持续时间以及皮肤划痕症评分差值以及总积分降低的幅度均高于对照组(t=12.271~39.332,P<0.05);并且研究组临床治疗总有效率也显著高于对照组(χ2=4.762,P<0.05);治疗后研究组IL-4、IL-10、IL-17、IL-23降低值和IL-35升高值均高于对照组(t=6.983~13.585,P<0.05);同时研究组患者CD4+T淋巴细胞水平升高值、CD4+/CD8+比值升高值和CD8+T淋巴细胞水平降低值均大于对照组(t=10.238~14.362,P<0.05)。结论血府逐瘀汤联合西替利嗪片治疗慢性荨麻疹疗效确切,可显著改善患者的临床症状,提高临床疗效,同时可调节患者细胞因子水平,改善免疫功能。
[Objective]To investigate the effect of Xuefu Zhuyu Decoction combined with cetirizine hydrochloride tablets on the serum levels of cytokines and T lymphocytes in patients with urticaria with blood stasis.[Methods]A total of 100 patients with urticaria with blood stasis who met the inclusion criteria were divided into control group and study group,with 50 patients in each group.The patients in the control group were given oral cetirizine hydrochloride tablets,and those in the study group were given Xuefu Zhuyu Decoction in addition to the treatment in the control group.The two groups were compared in terms of degree of pruritus,size of wheal,distribution of skin lesions,degree of skin erythema,number of skin lesions,and clinical outcome,as well as the serum levels of cytokines(interleukin-4(IL-4),IL-10,IL-17,IL-23,and IL-35)and T lymphocytes(CD3+,CD4+,CD8+,and CD4+/CD8+)before and after treatment.Results]Compared with the control group,the study group had significantly greater reductions in size and number of wheals,distribution of skin lesions,degree of pruritus,duration,skin scratch score,and total score after treatment(t=12.271-39.332,P<0.05).The study group had a significantly higher overall response rate than the control group(χ2=4.762,P<0.05).Compared with the control group,the study group had significantly greater reductions in IL-4,IL-10,IL-17,and IL-23 and a significantly greater increase in IL-35 after treatment(t=6.983-13.585,P<0.05).Compared with the control group,the study group had significantly greater increases in CD4+T lymphocytes and CD4+/CD8+ratio,as well as a significantly greater reduction in CD8+T lymphocytes(t=10.238-14.362,P<0.05).[Conclusion]Xuefu Zhuyu Decoction combined with cetirizine hydrochloride tablets has a good clinical effect in the treatment of chronic urticaria and can improve patients’clinical symptoms and clinical outcome,regulate the levels of cytokines,and improve immune function.
