摘要
目的了解玉溪市艾滋病检测实验室网络建设情况,评估实验室网络建设发展效果,为下一步防治艾滋病策略制定提供依据。方法回顾性分析玉溪市艾滋病检测实验室网络建设情况,从HIV抗体检测份数、病例早发现情况以及一年内死亡情况对其效果进行综合性评估。结果截止2016年底玉溪市已成立1家HIV确证实验室,16家HIV筛查实验室,124家快速检测点,覆盖全市所有乡镇,2004年至2016年筛查人次数占总人口比例由3.33%上升至21.37%(χ^2=1 294 747.946,P<0.001),历年阳性检出率呈下降趋势(χ^2=1 479.209,P<0.001),历年新报告HIV/AIDS病例一年内死亡率由9.74%下降至3.46%(χ^2=19.017,P<0.001),新报告HIV感染者在总报告HIV/AIDS病例中占比总体呈现下降趋势(χ^2=10.244,P<0.001)。结论玉溪市艾滋病检测实验室网络建设已具规模,HIV抗体检测量不断加大,及时检出大量HIV/AIDS并纳入规范随访、管理和抗病毒治疗,有效降低了患者死亡的风险。
Objective To understand the construction situation and evaluate the effect on HIV laboratory network in Yuxi prefecture,and provide a scientific reference for HIV/AIDS strategy adjustment.Methods Retrospective analysis was conducted to evaluated construction situation of laboratory network,there were three methods which were used comprehensive assessment what were HIV tested number,earlier diagnosis,and one year mortality after newly diagnose of HIV/AIDS.Results By the end of 2016,there were 1 HIV confirmatory laboratory,16 HIV screening laboratories and 141 HIV testing points covered all the villages and towns in Yuxi prefecture,the proportion of tested number among total population increased from 3.33%to 21.37%(χ^2=1294747.946,P<0.001),the HIV antibody positive ratio showed an downward trend year after year(χ^2=1479.209,P<0.001),the proportion of patients die in the first year after newly diagnosed were decreased from 9.74%to 3.46%(χ^2=19.017,P<0.001),the proportions of HIV cases among all the HIV/AIDS showed an downward trend year after year also(χ^2=10.244,P<0.001).Conclusion HIV test lab network construction was under well development,HIV testing was expanded,and a large number of HIV positive case were found out timely and incorporation into standardized follow-up,management and ART can effectively reduce the risk of death of patients.
