摘要
目的:探讨进展期胃癌经腹腔镜全胃切除D2淋巴结清扫术治疗的效果。方法:选取进展期胃癌接受全胃切除联合D2淋巴结清扫术治疗的患者86例,其中48例患者接受开腹手术治疗(对照组),38例患者接受腹腔镜手术治疗(观察组)。比较两组手术治疗情况及预后情况。结果:观察组手术时间长于对照组,术中出血量少于对照组,术后排气时间、胃肠功能恢复时间短于对照组,并发症发生率低于对照组(P<0.05);两组手术治疗过程中的肿瘤远近端切缘长度、淋巴结检出数量,术后生存率差异无统计学意义(P>0.05)。结论:腹腔镜全胃切除D2淋巴结清扫术可有效治疗进展期胃癌,减少术后并发症。
Objective:To investigate the effect of laparoscopic total gastrectomy plus D2 lymph node dissection in advanced gastric cancer.Methods:Eighty-six patients with advanced gastric cancer undergoing total gastrectomy combined with D2 lymph node dissection were enrolled and divided into a control group(48 patients,underwent open surgery)and an observation group(38 patients,underwent laparoscopic surgery).The surgical treatment and prognosis were compared between the two groups.Results:The operation time was longer,the intraoperative blood loss was less,the postoperative exhaust time and postoperative gastrointestinal function recovery time were shorter and the incidence of complications was lower in the observation group than the control group(P<0.05).There were no significant differences between the two groups in the length of the proximal and distal margins,the number of lymph nodes detected and the postoperative survival rate(P>0.05).Conclusion:Laparoscopic total gastrectomy plus D2 lymph node dissection can effectively treat advanced gastric cancer and reduce postoperative complications.
