摘要 目的应用MRI分析腮腺影像学表现与口干之间的关系,探讨2型糖尿病所致腮腺病的发病机制。资料与方法随机选取40~60岁的男性糖尿病患者59例和健康志愿者90例,采取MRI腮腺成像测量其体积、脂肪相对信号值(RSI),进行脂肪分型;同时采用口干问卷对口干和脂肪信号进行相关性分析。结果糖尿病组腮腺脂肪RSI为29.97±9.89,较对照组的24.79±8.06显著增高,差异有统计学意义(P=0.001);且与口干存在显著相关性(r=0.650,P=0.001)。本组59例糖尿病患者腮腺中,31例出现局灶成熟脂肪沉积,其中Ⅲ型24例、Ⅳ型7例。结论糖尿病所致腮腺病的发病与脂质或成熟脂肪浸润存在联系。脂质沉积可通过减弱唾液腺的分泌功能引起口干。 Purpose To investigate the pathogenesis of parotid gland disease caused by type 2 diabetes by analyzing the relationship between image finding and xerostomia using MRI.Materials and Methods Fifty-nine male diabetic patients aged 40-60 years and 90 healthy volunteers were randomly selected,and their volume and relative signal intensity(RSI)of fat were measured using MRI parotid gland imaging for fat typing.Then xerostomia questionnaire was used to analyze the correlation between xerostomia and fat signal.Results The RSI of parotid gland fat in the diabetic group(29.97±9.89)was significantly higher than that of the control group(24.79±8.06),and the difference was statistically significant(P=0.001);significant correlation with xerostomia existed(r=0.650,P=0.001).In the parotid glands of 59 diabetic patients,31 cases developed focal mature fat deposition,including 24 cases of typeⅢand 7 cases of typeⅣ.Conclusion Pathogenesis of parotid gland disease caused by diabetes is associated with lipid or mature fat infiltration.Lipid deposition may cause the occurrence of xerostomia by weakening the secretion function of the salivary glands.
