厄贝沙坦和咪达普利抑制自发性高血压大鼠左心室重塑的研究 被引量:3

Study of imidapril and irbesartan inhibiting remodeling of left ventricle in spontaneously hypertension rats

摘要 目的:探讨咪达普利、厄贝沙坦对自发性高血压大鼠(SHR)左心室重塑的抑制作用,并比较二者的作用效果。方法:选用13周龄的SHR30只、Wistar-Kyoto(WKY)大鼠10只,随机分为4组:SHR组,厄贝沙坦组,咪达普利组,WKY组。实验期15周。观察血压、左室重量/体重(LVW/BW)、左室厚度/BW,心肌的形态学(光镜、电镜),血浆、心肌血管紧张素Ⅱ(AngⅡ)及心钠素(ANF)浓度。结果:与SHR组相比,咪达普利组、厄贝沙坦组血压控制良好,LVW/BW、左室厚度/BW均比SHR组小(均P<0.05),血浆、心肌的ANF水平均比SHR组低(均P<0.001),咪达普利组血浆AngⅡ水平低于SHR组,但差异无统计学意义,心肌AngⅡ水平低于SHR组(P<0.05),厄贝沙坦组血浆、心肌AngⅡ水平均明显高于SHR组(均P<0.001),两组心肌结构改变尤其是纤维化均比SHR组减轻,咪达普利组减轻更明显。结论:咪达普利、厄贝沙坦不仅能良好地控制血压,而且可以抑制SHR左心室重塑;在防止心肌和肠系膜动脉结构改变尤其是纤维化方面,咪达普利作用可能优于厄贝沙坦。 Objective:To research effects of imidapril and irbesartan on inhibiting remodeling of left ventricle in spontaneously hypertensive rats and compare their effects. Methods: Thirty of 13 weeks old spontaneously hypertensive rats were randomly divide into three groups: SHR positive control group, imidapril treating group(3mg. kg-1 .d-1), irbesartan treating group(50 mg.kg-1d-1), ten homogenous Wistar-Kyoto rats were selected as normal control group. Monitoring blood pressure of rats periodically; after 15 weeks, putting all rats to death, taken out heart, measuring left ventricle weight(LVW) and thickness, then computing LVW/BW and left ventricle thickness/BW; Specimen of left ventricle was observed by light and transmission electron microscope; Ang II and ANF level of plasma and left ventricular myocardium were measured by radioimmunoassay. Results: Blood pressure and LVW/BW, left ventricle thickness/BW, ANF level of plasma and left ventricular myocardium of imidapril group and irbesartan group were lower than those of SHR group. Imidapril group and irbesartan group had no difference in these indexes; Ang II level of plasma, left ventricular myocardium in SHR group were same or lower that those in WKY group and the levels in irbesartan group was higher and in imidapril group were lower than that in SHR group; Left ventricle modulation in structure especially fibrosis was slighter in imidapril group and in irbesartan group. Furthermore, modulation of imidapril group was slighter than that of irbesartan group. Conclusions: Not only imidapril and irbesartan can controll blood pressure well but also they can inhibit remodeling of left ventricle in SHR; Imidapril is better than irbesartan in preventing left ventricle from modulation in structure especially fibrosis.
