摘要
目的研究成人腭大神经血管沟的解剖形态,探讨腭大神经血管沟的解剖形态与腭穹隆形态的相互关系,为涉及该位置的口腔临床操作提供理论依据。方法收集180名成人CBCT资料,记录左右侧上颌腭大神经血管沟的形态、位置范围,不同拱度腭穹窿下腭大神经血管沟分布情况。结果 (1) 94.12%的男性和94.74%的女性存在至少一条腭大神经血管沟,依照形态可分为凹陷型、带嵴凹陷型、双凹型及管状型四种类型;(2)腭大神经血管沟到上颌对应磨牙釉牙骨质界的距离统计结果为:男性显著大于女性(P<0.05);高腭弓大于低腭弓组(P<0.05)。结论成人腭大神经血管沟形态具有多样性,以凹陷型和带嵴凹陷型为主,其位置与腭穹隆高低和性别有相关性。CBCT可提供关于腭大神经血管沟的位置和形态信息,提高对该结构的认识,有助于区别上颌骨解剖及病理形态,为治疗方案的制定和临床诊断提供依据。
Objective To investigate the anatomical morphology of greater palatine groove(GPG) and its correlation with the palatine vault in the adults, so as to provide some data basis for the oral clinical operation related to this location. Methods Cone-beam computed tomography(CBCT) data of 180 adults were collected to record the morphology, distribution and location range of the GPG,and GPG on both the left and right sides of the maxilla, and to study the influence of different palatine fornix morphology on GPG. Results(1)At least one palatine canal was found in 94.12% of males and 94.74% of females,which could be divided into four types: a depressed type,a depressed with crest type,a double groove type and a tubular type.(2)The distances between GPG and CEJ of the corresponding molar were longer in males than in females(P < 0.05),and the distances were longer in the high palatal fornix group than in the low palatal fornix group(P < 0.05). Conclusion The morphology of GPG in adults is diverse,mainly in the depressed type and the deep groove with crest type. Its location is related to gender and the height of palatine vault. CBCT can provide information on the location and morphology of GPG,improve the understanding of the structure, help to distinguish the pathological morphology of the maxilla,and provide basis for the development of treatment plan and clinical diagnosis.
